The research background and significance:Oral squamous cell carcinoma(OSCC)is a common malignant tumor with high incidence rate and mortality rate.OSCC may occur anywhere in the mouth,but the most common are the tongue and the floor of the mouth.OSCC is easy to cause local infiltration and spread to cervical lymph nodes,with strong invasion and recurrence,resulting in poor prognosis and low survival rate.Early diagnosis is the premise of improving the cure rate and quality of life of OSCC patients.The development of biomarkers sensitive to OSCC at the molecular level is helpful to clarify the pathogenesis of OSCC,open up new diagnostic approaches,accurately judge the biological behavior of tumors,provide personalized treatment plans,evaluate the treatment effect of OSCC in real time and monitor the prognosis of OSCC,so as to reduce the recurrence rate of OSCC and improve the survival rate.Kn motif and ankyrin repeat domains 1(Kank1),also known as ankrd15,are important members of the kank family on chromosome 9p24.3.Kank1 is composed of three parts:N-terminal kn motif,conserved central coil domain and C-terminal anchor protein repeat domain.Kank1 is highly expressed in normal tissues such as heart,liver and kidney,and down regulated or even not expressed in many tumor tissues,which indicates that Kank1may be a tumor suppressor gene.At present,the expression and mechanism of Kank1 in OSCC have not been reported.Objective:To study the expression of Kank1 protein in normal oral mucosaand OSCC,as well as the relationship between expression level and clinical and pathological parameters such as differentiation degree,gender,age,lymph node metastasis,etc.in OSCC,to explore the possible pathway of Kank1’s involvement in the pathogenesis of OSCC,and to evaluate its value in the early diagnosis,prediction of the degree of malignancy,targeted precise treatment and prognosis indicators of OSCC patients.Methods:From the school of Stomatology,Jilin University,60 OSCC tissue samples and corresponding clinical and pathological data were collected,including 20 cases of high differentiation,20 cases of medium differentiation and 20 cases of low differentiation.Another 20 cases of NOM were selected as control.Immunohistochemical staining(IHC)was used to detect the expression of Kank1 in oral mucosa of OSCC patients and NOM.Combined with the clinical data of OSCC patients,the correlation between Kank1 and OSCC was studied.SPSS 24.0 statistical software was used to analyze the experimental data,andc~2 was used to compare the positive expression rate of Kank1.P<0.05 was used as the standard of statistical significance.Results:1.The expression level of Kank1 protein in OSCC was significantly lower than that in normal tissues(P<0.05).2.In high,medium and low differentiated OSCC,the expression of Kank1 was significantly decreased in turn,and there was a positive correlation between the expression of Kank1 and the differentiation degree of OSCC(P<0.05).3.Compared with the group of OSCC without lymph node metastasis,the positive expression rate of Kank1 in OSCC with lymph node metastasis was significantly lower,and the expression of Kank1 was negatively correlated with lymph node metastasis(P<0.05).4.In the OSCC group,there was no significant correlation between the expression level of Kank1 and the age and gender(P>0.05).Conclusion:The positive staining rate of Kank1 in normal mucosal tissues and different clinical pathological parameters of OSCC was detected by immunohistochemical methods.The results showed that the positive expression rate of Kank1 protein in OSCC was significantly lower than that in NOM tissues in the control group,indicating that there is a low expression of Kank1 protein in OSCC.The low expression of Kank1suggests that Kank1 may play a tumor suppressive role in OSCC,and its low expression or gene silencing may be involved in the pathogenesis of OSCC.The high expression rate of Kank1 in OSCC is positively correlated with the degree of differentiation,that is,the higher the degree of differentiation of OSCC,the lower the malignancy,the higher the high expression rate of Kank1,and the high expression rate of Kank1 protein was significantly higher in OSCC without lymph node metastasis than in OSCC with lymph node metastasis,suggesting that Kank1 may play a certain role in the occurrence,progression and metastasis of OSCC.Kank1 is expected to be an ideal indicator for assessing the progression and prognosis of patients with OSCC,and provide certain theoretical guidance for the early clinical diagnosis and more accurate targeted treatment of OSCC.
