Analysis Of The Efficacy Of Flexible Ureteroscope Lithotripsy Versus Percutaneous Nephrolithotomy In The Treatment Of Kidney Stones Larger Than 2 Cm In Patients With Solitary Kidney

Objective To compare the safety and effectiveness of flexible ureteroscope lithotripsy(FURL)and percutaneous nephrolithotomy(PCNL)in the treatment of kidney stones larger than 2 cm in patients with solitary kidney.Methods The clinical data of a total of 48 patients with solitary kidney suffering kidney stones with diameter> 2cm in our hospital from January 2014 to January 2019 were retrospectively analyzed,of which 27 patients received flexible ureteroscope lithotripsy(group F)and another 21 patients received percutaneous nephrolithotomy(PCNL),and all patients were operated successfully.The general conditions,operation time,preoperative and postoperative creatinine value,hemoglobin reduction value,total postoperative hospitalization time,and stone removal rate were analyzed,and these data were analyzed statistically.Results No significant difference was be found in the stone removal rate at three days after surgery(51.85% vs 71.43%,P = 0.169)and at one month after surgery(81.48% vs 85.71%,P = 1.000)and preoperative creatinine value(102.73 ± 28.31 vs 103.88 ±44.58?mol/L,P = 0.918)and after surgery creatinine value(94.56 ± 18.98 vs 89.96 ± 14.53?mol/L,P =0.346).In group F,the total postoperative hospitalization time(4.00 ± 2.84 vs 7.90 ± 2.72 d,P = 0.000),and the average hemoglobin reduction value(16.67 ± 14.47 g / L vs 7.44 ± 6.10 g / L,P = 0.004)were better than those in group P,but the average operation time in group F was longer than that of group P(95.37 ± 24.73 vs 72.14 ± 21.60 min,P = 0.001).Conclusion For the kidney stones larger than 2 cm in patients with solitary kidney,compared with percutaneous nephrolithotomy(PCNL),flexible ureteroscope lithotripsy(FURL)has advantages in postoperative hospital stay and bleeding,and the stone removal rate is comparable to PCNL.
