| Objective: To compare the efficacy of percutaneous nephrolithotomy(PCNL)and flexible ureteroscopic lithotripsy(F-URS)in the treatment of solitary kidney stones,and to choose the best operation method.Methods: A retrospective study,86 patients who suffering from solitary kidney stones in our hospital were selected,and 50 of them were selected.The diameter of stones were1.5-2.5cm.PCNL and f-urs can be used as surgical program.Percutaneous nephrolithotomy group(PCNL)26 cases,flexible ureteroscopic lithotripsy group(f-urs)24 cases,two groups of operation by the same operator to complete the whole operation,observe the two groups of patients with postoperative hemoglobin changes,length of stay,operation time,hospitalization expenses,one-stage stone clearance rate,postoperative complications.Results: There was no obvious difference in gender,age,past history,stone location,stone size,preoperative infection and the distribution of functional solitary kidney between the two groups(P > 0.05).Compared with F-URS group,PCNL group had higher postoperative hemoglobin decline:5.68+2.38vs2.20+0.90(P<0.05),longer hospital stay:7.79+1.63vs3.13+1.13(P<0.05),but lower total hospitalization cost :18028.21+1326.06vs24512.93+3357.65(P<0.05).The stone clearance rate of PCNL group was significantly higher than that of F-URS group:13(92.9)vs9(60.0)(P<0.05).However,there was no difference in the total stone clearance rate between PCNL group and F-URS group:14(100)vs13(86.7)(P>0.05).Conclusion: When the diameter of solitary kidney stones is between 1.5cm-2.5cm,and both operations can be used as the operation plan,the flexible ureteroscopic lithotripsy group(f-urs)has the advantages of rapid recovery of renal function,less intraoperative bleeding,short postoperative hospital stay,and no difference in stone clearance rate,which can be used as the preferred choice for minimally invasive surgery of solitary kidney stones. |
