ObjectivesBy analyzing the metabolic profile of serum bile acids in patients with different hepatobiliary diseases,the corresponding characteristic bile acids were screened in order to provide help for the diagnosis and differential diagnosis of common hepatobiliary diseases.MethodsFrom January 2018 to January 2019,a total of 218 patients with different hepatobiliary diseases and healthy physical examination in the first affiliated hospital of Chongqing Medical University were selected,which were divided into healthy control group(51 cases),hepatitis B-related liver disease group(21 cases of viral hepatitis B,37 cases of hepatitis B-related cirrhosis,37 cases of hepatitis B-associated hepatocellular carcinoma),cholelithiasis group(33 cases),alcoholic liver disease group(23 cases)and drug-induced liver damage group(16 cases).detect biochemical indicators on Roche C701.The OPLS-DA analysis were carried out to establish the clinical diagnosis model of different liverdiseases,to screen out the characteristic bile acids,and to analyze the statistical significance and diagnostic value of the selected characteristic bile acids.Results1.GCDCA,GCA,TDCA,TCA,TCDCA was the difference of bile acid in serum of hepatitis B-related liver disease group and normal control group.GCDCA,GCA,TCA,TCDCA(AUC>0.7)has good diagnostic value for hepatitis B and hepatitis B associated cirrhosis.ROC of hepatitis B-associated hepatocellular carcinoma is not typical,which needs to increase the sample size for further analysis.2.TCDCA、GCDCA、TDCA、GDCA、TCA、GUDCA、GCA was the difference of bile acids in serum of alcoholic liver disease,drug-induced liver damage and normal control group.Among them,TCDCA,GCDCA,TDCA,TCA,GCA(AUC>0.7)has good diagnostic value for alcoholic liver disease.GDCA(AUC=0.674)has certain diagnostic value for drug-induced liver damage.but there was no significant difference in bile acids between alcoholic liver disease and drug-induced liver damage,suggesting that alcoholic liver disease and drug-induced liver damage have a common bile acid metabolic pathway.3.GCDCA、CDCA、GUDCA、TUDCA、GCA、TDCA、TLCA、TCDCA、TCA was the difference of bile acids in serum of cholelithiasis and normal control group.Compared with healthy control group,thedifference of GCDCA、CDCA、TUDCA、GCA、TLCA、TCDCA、TCA in cholelithiasis group was statistically significant(P<0.05).The AUC of TLCA in cholelithiasis group was 0.615(P<0.05),and it was considered that TLCA had better diagnostic value for cholelithiasis.ConclusionsHepatobiliary diseases with different causes have the corresponding characteristic bile acids.The metabolic profile of bile acids is expected to be used in the diagnosis and differential diagnosis of different hepatobiliary diseases.
