Clinical Observation Of The Curative Effect Of Modified Duhuo Jisheng Decoction On The Swelling And Pain After Total Knee Arthroplasty

Objective: By observing and assessing the curative effect of the swelling and pain,the incidence of chronic pain and the knee function score after oral modified Duhuo Jisheng decoction on the basis of routine treatment and functional exercise by patients who finished a total knee arthroplasty,in order to provide certain references and basis for the clinical optimization of the treatment and rehabilitation of patients after total knee arthroplasty.Methods: The cases were collected from the number of 60 elderly patients that were admitted to the 6th division of the orthopedics department of the Fuzhou Second Hospital of Fujian province from December 2018 to September 2019,whose preoperative diagnosis was consistent with knee osteoarthritis,possessed surgical indications and were underwent unilateral total knee arthroplasty by the same group of physicians.The patients were numbered and randomly divided into the control group and the treatment group,with 30 patients in both groups.The control group prescribed the patients with routine treatment and functional exercise,the treatment group prescribed the patients with modified Duhuo Jisheng decoction on the basis of the treatment plan of the control group and continued for 3 months.Visual analogue scale(VAS)and knee swelling degrees were recorded before the surgery,and the 1st day,the 7th day,2 weeks,1 month and 3 months after the surgery,with the calculations of the incidence of chronic pain in both groups at three months after the surgery.Hospital for special surgery knee score(HSS)were recorded before the surgery,the 7th day,2weeks,1 month and 3 months after the surgery.The recorded data were compared and analyzed in the final.Results: 1.Respectively 26 and 27 patients were followed up and completed 3 months of treatment in the control group and the treatment group.There was no statistical difference in the general data of the two groups(P>0.05).2.VAS score: There were no statistical differences in the data between the two groups that were recorded before the surgery and the 1st day after the surgery(P>0.05).By comparing the data recorded on the 7th day,2 weeks and 1 month after the surgery,the VAS score of the treatment group was lower than the control group,which was statistically significant(P<0.05).There was no significant difference between the data recorded of the two groups 3 months after surgery(P> 0.05).3.The incidence of chronic pain: The incidence of chronic pain in the control group was38.5%,and the incidence of chronic pain in the treatment group was 11.1%.The difference between the two groups was statistically significant(P<0.05).4.Swelling of the knee joint: Preoperative comparison and postoperative comparison of the two groups before the surgery and on the 1st day after the surgery of the circumference at 10 cm above the upper pole of the patella and 10 cm below the lower pole of patella showed no statistical difference(P>0.05).The differences between the degree of swelling at 10 cm above the upper pole of the patella and the degree of swelling at the 10 cm below the lower pole of the patella on the 7th day,2 weeks and 1 month after the surgery all showed that the treatment group was lower than the control group,which was statistically significant(P<0.05).Although there was a difference between the two groups at 3 months after the surgery,it was not statistically significant(P>0.05).5.HSS score: There was no significant difference between the two groups in preoperativeHSS score(P>0.05).The HSS score of the two groups on the 7th day,2 weeks,1 month and 3months after the surgery showed the treatment group was better than the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusion: The oral of modified Duhuo Jisheng decoction on the basis of routine treatment and functional exercise possessed quickly and effectively elimination to the swelling and pain of patients after total knee arthroplasty,also contribute to the patients’ knee joint function recovery,reduce the incidence of chronic pain,conduce to improve the quality of daily life and the satisfaction of curative effect of patients after total knee arthroplasty,which can suggest to apply and promote in clinical condition.
