Objective:Cancer and its treatment can affect the physical fitness of cervical cancer survivors,produce cancer-related fatigue(CRF),reduce the quality of life,and thereby increase the recurrence and mortality rate of patients.Previous research has shown that exercise can effectively improve the physical fitness and inflammation of cancer patients,thereby alleviating the symptoms of fatigue and improving the quality of life.However,many studies have not yet reached a consensus on which exercise method can most effectively improve the physical condition of cervical cancer survivors.This study intends to find a more effective economical,convenient and feasible rehabilitation method for cervical survivors by comparing the effects of aerobic combined resistance exercise and single aerobic exercise on fatigue,quality of life,physical fitness and inflammation in cervical cancer survivors.Method:Cervical cancer survivors who were treated in the Department of Gynecologic Oncology,Beijing Obstetrics and Gynecology Hospital,Capital Medical University were recruited as the subjects.Eligible 48 subjects were randomly divided into control group(CG),aerobic exercise group(AG)and aerobic combined resistance exercise group(ARG).The subjects of AG and ARG performed aerobic exercise and aerobic combined resistance exercise for 12 weeks respectively.The subjects of CG maintained the lifestyle without any exercise intervention as usual.A total of 41 people completed the research,of which 13 were CG,14 were AG,and 14 were ARG.The subjects of AG and ARG exercise last about 70 min,including 10 min warm-up exercise,50 min exercise intervention and 10 min stretching exercise.Participants were trained once a week at the Institute of Sports Medicine of the State General Administration of Sport,and the rest 2 times were performed at home.The method of aerobic exercise was brisk walking,exercise intensity was 55%-70%HRmax,three times a week,of which aerobic exercise time of AG was 50 min,aerobic exercise time of ARG was 30 min.The ARG subjects also added elastic band resistance training,with an exercise intensity of RPE 11-13 and a total of 9 sections.Each section was in 2 groups,8-10 times each,with a 1-3 minute rest between the groups.Twice a week,did the exercise after aerobic exercise.Subjects were tested and data collected one week before the exercise intervention after the intervention.The subjective indicators include cancer-related fatigue(Piper fatigue revised scale)and quality of life indicators(EORTC QLQ-C30).The objective indicators include physical fitness(body composition,cardiopulmonary function and muscle function)and inflammation indicators.Results:1.Cancer-related fatigue and quality of lifeAfter 12 weeks of exercise intervention,the RPFS of AG decreased significantly(P<0.05),and the RPFS of ARG decreased significantly(P<0.01).Comparing the differences of the three groups,it was found that compared with CG,the improvementtrend of AG was significant(P<0.05),and the improvement trend of ARG was more significant(P<0.01).After 12 weeks of exercise intervention,the EORTC QLQ-C30 of AG and ARG were significantly improved(P<0.01).Comparing the differences of the three groups,it was found that AG and ARG were significantly improved compared with CG(P<0.01).2.Physical fitnessAfter 12 weeks of exercise intervention,the body weight,BMI,and body fat rate of AG were significantly lower than those before the intervention(P<0.05).The body weight and BMI of ARG were significantly reduced(P<0.05),the body fat rate and muscle mass were significantly improved(P<0.01).Comparing the differences of the three groups,it was found that compared with CG,the weight,BMI,and body fat rate of AG decreased significantly(P<0.01).The weight and BMI of ARG decreased significantly(P<0.05),and muscle mass increased significantly(P<0.01).Compared with AG,the body fat rate of ARG decreased significantly(P<0.05),and the muscle mass increased significantly(P<0.01).After 12 weeks of exercise intervention,the VO2 peak and METS of AG and ARG were significantly increased compared with those before intervention(P<0.01).Comparing the differences before and after the three groups,it was found that compared with CG,the VO2 peak and METS of AG and the VO2 peak of ARG increased significantly(P<0.05).After 12 weeks of exercise intervention,the 30-STS and TUG in the AG were significantly improved compared with those before intervention(P<0.05).ARG showed significant improvement in grip strength,30-AC,30-STS and TUG(P<0.01).Comparing the differences of the three groups,it was found that compared with CG,the TUG of AG improved significantly(P<0.05),and the 30-AC,30-STS,and TUG of ARG showed significant trends(P<0.01).Compared with AG,30-AC,30-STS and TUG of ARG improved significantly(P<0.05).3.InflammationAfter 12 weeks of exercise intervention,there was significant reduction of TNF-α in the ARG.Comparing the differences of the three groups,it was found that compared with CG,the reduction of TNF-α of ARG was significant(P<0.05).Conclusions:12 weeks of aerobic combined resistance exercise can effectively improve the physical fitness of cervical cancer survivors,reduce inflammation,relieve cancer-related fatigue and improve quality of life.12 weeks of aerobic exercise can improve the physical fitness of cervical cancer survivors to a certain extent(without significant improvement in muscle mass,upper limb muscle strength and proinflammatory cytokines),improve cancer-related fatigue and quality of life.Aerobic combined resistance exercise for cervical cancer survivors after treatment is safe and effective.Compared with single aerobic exercise,aerobic combined resistance exercise can bring extra benefits to survivors.Therefore,aerobic combined resistance exercise can be one of the effective methods for cervical cancer survivors to relieve cancer-related fatigue and improve quality of life.
