China is facing the current situation of poor doctor-patient relationship and social trust.How to build good doctor-patient relationship has always been a social problem.In view of this topic,the author first through the literature analysis and the current situation research,found that the current research on the characteristics,mechanisms,factors and other factors of the relationship between doctors and patients is more thorough,but the quantitative research on the characteristics of the mechanism of doctor-patient relationship is still relatively few;the local research on the patient’s medical treatment is a partial study.More,the comprehensive and continuous research is still relatively small;the doctor-patient trust is regarded as a medical treatment result,and the study of trust as a result of the changes in the relationship between doctors and patients is less.However,trust is the basis for building a harmonious doctor-patient relationship,and is also an important guarantee for achieving the best medical results.through the meta analysis,this paper systematically evaluates the characteristics of the factors affecting Chinese medical disputes,and finds that iatrogenic factors are still the main factors in the occurrence of medical disputes in China.The difference of the patient’s own expectation is the important factor in the occurrence of medical disputes in China,and the social factors aggravate its development.On this basis,the research group chose obstetrics and pediatrics as the research objects,which are very vulnerable to doctor-patient disputes,and explored the current status of doctor-patient relationship in this group.The results showed that pediatric obstetrics,as a profession more vulnerable to medical disputes,had an average score of 28.87 on the relationship between physicians and patients with group difficulties.Although it was less than the total score of 35 points for the subjects in choosing uncertain items,the relationship between doctors and patients was in a relatively moderate degree,but it was not in a better state.The degree of burnout has a significant negative impact on the doctor-patient relationship;the longer the working week,the lower the evaluation of the doctor-patient relationship.In order to further explore the influence mechanism of doctor-patient relationship,the research group aims at the difficulty that "doctor-patient relationship contains many components and is constantly changing,and it is difficult to generalize by measuring a variable or a time node".The article introduces "multistage patient trust" throughout the whole process of medical treatment,and through the change of this component,it shows the impact on other components.It is doubtful that the doctor-patient relationship can be described more comprehensively and systematically.Through a follow-up survey of 200 patients in a large third-level hospital in southwest China,the influencing factors of patients’ initial trust were explored by single factor analysis of variance and multiple regression analysis,and the relationship between initial trust,perceived value and follow-up trust,patient satisfaction and patient loyalty was explored by structural equation modeling.The results showed that patients’ initial trust before seeking medical treatment was mainly influenced by factors such as hospital reputation and hospital scale;patients’ initial trust positively affected patients’ follow-up trust,but it could not regulate the effect of patients’ perceived value on patients’ satisfaction;patients’ satisfaction was part of the mediating variable between patients’ perceived value and follow-up trust;patients’ satisfaction was patients’ perceived value and patients’ loyalty.Some mediating variables,and the mediating effect is far greater than the direct effect.The research suggests that:1.Hospital managers should understand the relationship between doctors and patients in a dynamic and complex way,find out the main contradictions existing in the current relationship between doctors and patients,and patients’ perceived value positively affects many important components of the relationship between doctors and patients,which is the key link to improve the relationship between doctors and patients.2.Health administrative departments should strengthen the overall planning of health resources,strengthen the training of grass-roots medical institutions,establish a reasonable hierarchical diagnosis and treatment system,seek "open source" of medical services for patients,and "reduce burden" for tertiary hospitals.3.It is necessary to strengthen the training of pediatricians and obstetricians and increase relevant investment in order to alleviate the heavy burden of pediatricians and obstetricians and reduce the work pressure of existing doctors.
