Since the beginning of the 21 st century,with the progress of social economy,the state has paid more and more attention to people’s health.At the 19 th National People’s Congress of the communist party of China in 2017,state leaders directly identified people’s health as an important symbol of national prosperity and national prosperity.In order to disperse the economic risks caused by diseases among the public,maintain the normal living standard of the nation,and promote the health of the people and the prosperity of the country,China has further promoted the integration of the basic medical insurance system for urban and rural residents.After the system integration,the original bad phenomenon has been improved to a large extent,highlighting the fairness of the social health security system.However,this fairness is reflected in the vertical comparison between the new system and the old system,rather than the horizontal comparison of the implementation of the new system in different regions.On the contrary,in the less developed areas of China,there are still many problems in the implementation of the basic medical insurance system in urban and rural areas,resulting in no significant improvement in the social health security level of residents in the less developed areas.Even due to the lack of understanding of the new system and other factors,the participation rate of basic medical insurance in urban and rural areas has decreased in some underdeveloped areas.This is not conducive to social stability and the overall improvement of residents’ health level,so it is worth further study to solve.Based on this,this paper adopts case study method,field investigation method and other research methods,taking P township,Shangsi county,Guangxi province as the case study,and combining the theory of public goods and social equity theory and other theoretical basis,to study and analyze the decline of the insurance participation rate of farmers in underdeveloped areas in China under the background of the integration of urban and rural medical insurance.The research focuses on the development of the basic medical insurance system for urban and rural residents of P township and the current situation of residents’ participation in the insurance,analyzes the main reasons leading to the decline of the residents’ participation rate in this area,and puts forward some Suggestions to solve the problem.The research results show that the problem of medical insurance participation rate of urban and rural residents exists in P township every year,which can be directly reflected from the data over the years.The formation of this problem is related to many factors,mainly due to the low medical insurance benefits of ordinary residents,the service capacity of primary medical institutions is not strong,the government’s propaganda on the system is not in place,and there are four structural imbalances among the insured groups.In order to solve the problem of declining insurance participation rate and improve the social health security level in underdeveloped areas,it is suggested that relevant government departments and medical institutions make improvements,improve the medical insurance treatment of residents,strengthen the service capacity of medical institutions,adjust and optimize the structure of the insured group,and strengthen the propaganda of the government system and so on.
