| Objective To further explore the clinical value of Zero-P(Zero-profile Integrated Late and Spacer Device)in the treatment of cervical spondylosis by comparing the application and analyzing the clinical efficacy of anterior cervical intervertebral fusion and fixation between the traditional titanium plate +Cage group and the Zero-P group.Methods The clinical data of cervical spondylosis patients treated in the Spinal Surgery Dept.of our hospital from January 2015 to June 2019 were retrospectively collected and analyzed.All patients underwent surgical treatment.The enrolled patients were treated with anterior cervical discectomy +cervical intervertebral fusion surgery.Fifty-nine patients were included and divided into the Zero-P group(34 cases)and the traditional titanium plate+Cage group(23 cases)according to the different choice of internal fixation system.All patients were followed up for at least 1 year after surgery.The surgical time,intraoperative blood loss were compared,and the VAS,JOA,and NDI scores were measured preoperative,1 month 3 months,6 months,and 12 months after the surgery to evaluate the clinical efficacy.The fusion status was evaluated by measuring the Cobb angle of the lateral radiographs before,3 months,1years after the surgery,and conducting cervical X-ray of hyperextension and hyperflexion or CT scan.The incidence of surgical complications was compared.Results Symptoms improved in all patients.No significant differences were found between the two groups in gender,age,operation time,surgical blood loss,preoperative VAS score,JOA score,and NDI score.Repeated measures analysis were carried out in VAS,NDI,and JOA scores and Cobb angle,and no significant difference were found in the them.The postoperative time affected the them with clinical improvement of all them in patients underwent different surgery.No interaction exists between the surgical method and the postoperative time.The variation of the three scores and Cobb angle in the two groups was roughly the same as the postoperative time prolonged.No significant difference in osseointegration rate at the surgical segment at 1 year after operation.The incidence of surgery-related complications,including dysphagia,showed no significant difference between the two groups.Conclusion The application of Zero-profile Cervical Anterior Cervical Interbody Fusion(Zero-P)showed satisfied early-stage therapeutic effects in the treatment of internal fusion and fixation in cervical spondylosis patients.Zero-P,as a stable and reliable method,can effectively improve and maintain the curvature of cervical with high fusion rate,is one of the safe and effective methods to substitute the traditional titanium plate + cage fusioninternal fixation.However,longer-term follow-up is still needed to further evaluate its function and impact on adjacent segments disease. |
