Objective :We aimed to compare the efficacy and safety of Dutasteride and Finasteride in the treatment of transurethral resection of prostate provides a better theoretical basis for the selection of perioperative and postoperative drug therapy for prostatic hyperplasia. Methods:The retrieval databases included PubMed,EMBASE,Scopus,Cochrane Library,CNKI,VIP and WanfangData.Clinical trials of the above two drugs(Dutasteride and Finasteride)on the treatment of blood loss by transurethral resection of prostate were comprehensively collected from the establish to April 2019.Two researchers independently conducted literature screening, data extraction and methodological quality evaluation, and adopted RevMan5.3software for Meta-analysis.Results:A total of 9 randomized controlled studies including 1636 patients were included in this study, including 814 in the duasteride group and 822 in the finasteride group. The results of meta-analysis showed that:(1)general information:there were no statistically significant differences in prostate volume, maximumurine flow rate and IPSS score between the two groups(P >0.05).(2)intraope rative indicators: after taking two drugs before surgery, there were statistically significant differences in intraoperative blood loss, blood loss pergram of prost ate tissue and microvascular density(MVD)between the two groups(P < 0.05).There was no significant difference between the two groups in hemoglobin difference before and after surgery, hematocrit tolerance, operation time, and intraoperative bladder rinse fluid(P >0.05).Conclusion:The experiment has shown that duasteride is more significant than finasteride in reducing intraoperative blood loss, loss of blood pergram of prostate tissue, and microvascular density(MVD).However, there is no significant difference between the two drugs in reducing hemoglobin and hematocrit and in shortening the duration of surgery. To some extent, preoperative use of duasteride is more able to reduce TURP bleeding.
