| Objective To explore the clinical significance of preoperative BOLD-fMRI combined with DTI and white matter fiber bundle reconstruction in guiding the design of cortical and subcortical surgical pathways in deep glioma.Methods 20 patients with deep brain glioma were diagnosed as subjects.Routine MRI 、 BOLD-fMRI and DTI examination were performed before operation to locate the anatomical location of cerebral cortical functional area and the 3D reconstruction of cerebral white matter fiber bundle under cerebral cortex.The relationship between cerebral cortical functional area and white matter fiber bundle and deep brain glioma was clarified in order to avoid cerebral cortical functional area and important areas brain white matter fiber bundle principle to design surgical pathways.Results clarify the three-dimensional spatial relationship between the anatomical location of functional area and the white matter fiber bundle of brain,17 cases of total tumor resection and 3 cases of secondary total resection were performed under microscope.The BOLD and DTI of postoperative reexamination showed that the functional area of cerebral cortex and the important white matter fiber bundle of the brain were well preserved,and the postoperative KPS score of the patients was significantly higher than that before operation.Conclusion Preoperative BOLD-fMRI combined with DTI examination and reconstruction of white matter fiber bundle are important for removing deep gliomas.It can protect the white matter fiber bundle and important cortical area and reduce the operation after the rate of disability,improve the quality of life of patients. |
