Study On Management Mechanism Of The Blood Collection And Supply Information System In W City

Research objective:Based on the government responsibility theory and the information society theory,this research investigated the current management situation of blood collection and supply information system of W city in order to understand the problems and cause of the management of blood collection and supply information system in W city.On this basis,this research aims to explore the countermeasures to build and improve the management mechanism of blood collection and supply information system in W city unified China’s national condition.Research methods:1.Documentation analysis method: Literature and policy reports on blood collection and supply information system management at home and abroad were collected through consulting Chinese Selected Doctoral Dissertations Master’s Theses Full-Text Database(CDMD)、 Chinese Journal Full-Text Database(CJFD)、 VIP Journals Database and books related to information management.On the basis of extensive reading,the research analyzed,sorted and summarized results of blood collection and supply information management.2.Expert consultation method: Experts in the field of blood collection and supply information management and health management were chosen to provides suggestions on the design rationality of the interview outline,and to demonstrate the construction and perfection of the management mechanism of blood collection and supply information system in the later stage.3.In-depth interviews: A semi-structured interview outline were used to interview the heads of the information departments of the four blood collection and supply institutions in W city,understanding the information security,authority settings,system maintenance,responsible subjects of the blood collection and supply institutions,clarifying the problems and affecting factors in information system management,and providing advice on the construction and improvement of information system management mechanisms in its later stage.4.Normative analysis: Based on the collected interview information and literature materials,the author used multidisciplinary theories to make logical reasoning and combined with China’s national conditions to seek ways and means to improve the management mechanism of the blood collection and supply information system in W city.Research results:1.Operation and guarantee of blood collection and supply information system in W city In January 2019,W city had established an information management system covering the blood collection and supply institutions in the whole city,which consists of four sections: blood allocation,blood donor shielding,blood expense reimbursement and comprehensive blood administrative management.At present,the internal information systems of each blood collection and supply institutions had been connected with the municipal information system,and the two systems of each institution were running simultaneously.(1)Responsibility body of management of municipal blood collection and supply information system: The responsibility body for the management of the municipal blood collection and supply information system was the medical administration and medical management of the health commission of W city,which was responsible for the comprehensive administrative responsibilities such as the examination and approval of blood allocation.The blood center of W city was responsible for the operation,maintenance and management of the system.(2)Information management staff of blood collection and supply information system: There were 36 information management staff in 16 blood collection and supply institutions of W city,among which there were only 15 full-time staff,accounting for 41.67%,and 21 part-time staff,accounting for 58.33%.The information management staff of the municipal centre blood bank and the county centre blood bank all had computer professional degrees,while the blood collection spot only had clinical medicine graduate.(3)Blood collection and supply information system funding guarantee:The municipal blood collection and supply information system was fully funded by the municipal government of W city,and the system was maintained free of charge by the software development company for the first 3 years.Each blood collection and supply institution should pay for the operation and maintenance of the internal information management system.(4)Blood collection and supply information system equipment and facilities: The machine room of the municipal blood collection and supply information system was constructed according to the B-class machine room standard in the national standard "Design Code for Electronic Information System Machine Room",which was set in the blood center with an area about 80 square meters.The machine room was equipped with necessary equipment such as temperature and humidity monitoring,lightning protection system,gas fire control and precision air conditioning,but the construction and equipment of the machine room of each blood collection and supply institution had not reached the national standard.2.The management of blood collection and supply information system in W city(1)Blood collection and supply information management: The management section of blood collection and supply business had not been opened,so the management of the whole business chain such as blood collection,preparation,testing,scrapping,storage and delivery could not be realized.The recruitment board for blood donors had not been opened,so it was impossible to make blood donation appointment,blood donation records and blood test results.(2)Information security management: The municipal blood collection and supply information system had reached the third level of national information security protection;According to the staffs’ levels and functions set different scope of authority,and the account and password dynamic management.Meanwhile,The data of the system could not be edited,deleted and other operations.(3)System emergency management: The data of the municipal blood collection and supply information system were not uniformly backed up,and the original data were still kept by each blood collection and supply institutions.The emergency management system had not yet been formed,lacking of the emergency management responsible institutions,the corresponding responsibilities and authorities,collaboration among blood collection and supply institutions,the reporting process,the emergency process,etc.At present,only the W city’s blood center information staff had stimulated a few times of outage and server outage drills.3.Collaboration and management between agencies(1)The cooperation among blood collection and supply institutions: The blood collection and supply institutions in W city could share the information of blood donors.As long as the blood collection points were connected with the municipal blood collection and supply information system,they could shield inappropriate blood donors timely during blood collection and avoid the waste of human,material and financial resources.The blood donor shielding section mainly played the role of blacklist shielding.However,there were no unified provisions and standards for the blood test reagent and no specific shielding time limit varying with the test results in various blood collection and supply institutions of W city.The blood allocation section mainly provided information for the blood allocation of blood collection and supply institutions.However,the blood allocation section on the system was not used,and it was still a paper application for allocation.Meanwhile,the coding conversion must be required in the actual allocation operation due to different blood coding information standards.(2)Collaboration between blood collection and supply institutions and hospitals: The cooperation mainly involved the section of blood expense reimbursement in the municipal blood collection and supply information system to realize the reimbursement and unified settlement of unpaid blood donors in different places.However,the system had not yet connected the network of hospital,and the section had not been put into use.In addition,the system did not develop hospital clinical blood transfusion information plate,so the blood collection and supply institutions and hospitals could not share information such as blood grouping,cross-matching,antibody screening,adverse effects of blood transfusion and blood inventory,at the same time,the blood reserve function could not achieve.(3)The management of blood collection and supply institutions and hospitals by health administrative department: The medical administration and medical management department of the health commission of W city could inquire and monitor the blood information of each blood collection and supply institutions and hospitals,meanwhile conducted business approval through the municipal blood collection and supply information system.Research conclusions:1.Problems in operation and guarantee of blood collection and supply information system of W city The responsibility body of management of the municipal blood collection and supply information management system and the regulations of rights and responsibilities were unclear;There were few full-time staff in blood collection and supply institutions;The the blood collection spot was lack of computer professionals;Due to the cancellation of blood compensation,there was great difficulty in the operation and maintenance of information system for the blood supply and collection institutions,which had been funded by balance allocation and own revenue and expenditure;The facilities and equipment of computer rooms in the blood collection and supply institutions not reached national standards,and there were a security risk in the blood collection and supply information.2.Problems in the management of blood collection and supply information system of W city The municipal blood collection and supply information system had not opened the functions of blood collection and supply business management,hospital clinical blood transfusion management and blood donor recruitment management;The blood data were not backup,and data recovery was difficult in disaster;The perfect contingency plan had not been formed,and the ability to deal with the system security risks was low.3.Problems in collaboration and management between agencies The blood collection points were not all connected with the blood information management system,so the unsuitable blood donors could not be shielded;Shielding standards and blood coding standards were not unified among blood collection and supply institutions;The function of the blood allocation section in the municipal blood collection and supply information system was not played,the W city’s blood allocation and linkage mechanism was not sound;The municipal blood collection and supply information system lacked corresponding functional sections,so the health administrative department could not monitor and query all blood information of blood collection and supply institutions and hospitals.Policy suggestions:1.The improvement of blood collection and supply information system of W city: All blood collection and supply institutions in W city abolish the use of internal blood information system,and unify the use and improvement of municipal blood collection and supply information system,which is composed of four sections: blood collection and supply business management,hospital clinical blood transfusion management,administrative comprehensive management,and public information management.We include blood donor shielding plate into the blood collection and supply business management plate,the blood expense reimbursement plate into the hospital clinical transfusion management plate,and the blood allocation plate into the administrative comprehensive management plate.2.Operation and guarantee of blood collection and supply information system in W city(1)Define the responsibility body of the management of municipal blood collection and supply information system: The health commission of W city,the responsibility body,entrusts the W society of blood transfusion to management.At the same time,the responsibility,the relevant rights and the corresponding punishment measures of the W society of blood transfusion should be clearly regulated.(2)Strengthen the construction of talent team: Introduce and train IT talents.(3)Information system funding guarantee: The operation and maintenance expenses shall be borne by the finance of W city.(4)Information system equipment and facilities: the maintenance cost in the later period shall be borne by the financial department of city W.3.Improve the management of blood collection and supply information system of W city(1)Blood collection and supply business management: The blood collection points should connect with the municipal blood collection and supply information system to screen the unsuitable blood donors;The health commission
