| Objective:1.To understand the effect of obstructive sleep apnea hypopnea syndrom(obstructive sleep apnea hypopnea syndrome,OSAHS)caused by intermittent hypoxia(intermittent hypoxia,IH)on serum(C(cystatin C,CysC)inhibition.2.To detect the level of CysC in patients with OSAHS complicated with cardiovascular disease and investigate whether it has predictive significance for endothelial injury caused by sleep apnea IH.3.To observe the therapeutic effect of continuous positive air pressure(CPAP)on OSAHS patients with cardiovascular disease.Methods:From handan city east central hospital respiratory medicine selection from September 2017 to September 2019 menstrual polysomnography(polysomnography,PSG)diagnosed with a total of 96 patients with OSAHS and detection of serum CysC:(1)general information collection,guide more sleep test index,blood sugar,blood fat,such as by multi-factor analysis,understand the OSAHS patients caused by IH effects on serum CysC.(2)selected from 96 patients diagnosed patients with cardiovascular disease,and kidney,lung,liver,and other important organs except organic lesions in 22 cases as the observation group,select the general information and apnea hypoventilation index(apnea hypopnea index,AHI)that match the 22 patients with OSAHS alone as control group,compared two groups of patients in serum CysC,the differences between the indexes of blood sugar,blood fat,understand about whether serum CysC OSAHS combined cardiovascular disease have predictive value.(3)A total of 14 patients in the observation group were treated with CPAP.The changes of serum CysC before and after 1 week of treatment were compared to investigate the therapeutic effect of CPAP on OSAHS patients with cardiovascular disease.Results:1.Serum CysC was positively correlated with AHI,SIT90,BMI,LDL,TC,and negatively correlated with MSpO2 and LSpO2,with statistical significance(P<0.05).Multiple linear regression showed that AHI was an independent risk factor for CysC.2.Compared with the control group,the serum CysC in the observation group was 2.01±0.71mg/L,and that in the control group was 1.54±0.63mg/L.The difference in serum CysC between the two groups was statistically significant.3.The serum CysC level of patients in the observation group was 1.78±0.77mg/L before CPAP treatment,and 0.90±0.16mg/L after 1 week of treatment,with statistically significant changes before and after treatment.Conclusions:1.Nocturnal intermittent hypoxia in OSAHS patients has an important effect on serum CysC.2.Serum CysC has predictive significance for endothelial injury in patients with OSAHS complicated with cardiovascular disease.3.CPAP treatment is significant in improving the treatment of OSAHS patients with cardiovascular disease. |
