Objective The purpose of the research work of this paper is mainly to deeply study the clinical diagnosis and treatment methods of ovarian malignant tumors in our country:The important clinical role of CA125,HE4,ROMA,B-ultrasound and MRI in the study of the early pathological changes of ovarian malignant tumors and their diagnostic and therapeutic value,so as to provide reliable basis and reference for clinical diagnosis and treatment.Methods Retrospective analysis The patients admitted to the Department of Gynaecology of Gansu Provincial Hospital from February,2018 to March,2019.the initial diagnosis was ovarian mass.Preoperative serum CA125,HE4 and ROMA were collected in combination with preoperative MRI and B-ultrasound for the preliminary diagnosis of benign and malignant ovarian masses.The pathological results of postoperative paraffin sections were used as the criteria for comparison.Individual and joint diagnosis to calculate its sensitivity,specificity,accuracy and predicted value,etc.Results 1.The serum CA125,HE4 and ROMA indexes in the ovarian malignant tumor group were compared with those in the benign ovarian disease control group,and the differences were statistically significant(P=0.000,0.001,0.000,P=0.000<0.05).There were statistically significant differences between the ovarian cancer group and the female health control group(P=0.000,0.001,0.000,P=0.000<0.05).There were no statistically significant differences in serum CA125,HE4 levels and ROMA index between the benign ovarian disease group and the female health control group(P=0.081,0.792,0.664,P>0.05).2.The independent detection of CA125,HE4 and ROMA is of great significance in the diagnosis of ovarian tumors.The AUCROC in the malignant group were 0.864,0.819,and0.841,and the AUCROC in the benign group were 0.719,0.702,and 0.727,respectively.The area under the curve in the malignant group was>0.8,and the area under the curve in the benign group was<0.8,indicating that The CA125,HE4 and ROMA are used alone to detect ovarian malignant tumors,and all have high statistical significance.The data of the malignant group were compared in pairs,that is,the P values of the three groups were all=0.000,0.000and 0.000,and the P values of the three groups were all<0.05,that is,there were statistically significant differences between the three groups.3.The expression levels of CA125 and HE4 before and after menopause were investigated.In this article,before and after menopause,there was no significant difference in serum CA125 and HE4 levels in the ovarian malignant tumor group(P>0.05).In the benign ovarian disease group,there was a significant difference in serum CA125 levels before and after menopause,and the difference was statistically significant(P<0.05).It can be seen from the results that there is no significant difference in serum HE4 levels in the malignant tumor group before and after menopause,(P>0.05).It can be seen that the serum HE4 level in the ovarian malignant tumor group remains stable before and after menopause.Whether menopause has little effect on HE4.4.B-ultrasound combined ROMA test results showed that the combination of the two significantly improved the diagnosis of Se and Sp compared to the single test,up to 84.2%,82.4%.At the same time,the DAR joint detection is also significantly improved compared to the single detection,reaching 83.0%.The results of MRI combined with ROMA showed that the combination of the two significantly improved Sp and PPV compared with the single detection,up to 84.0%and 81.0%.5.Comparing the diagnostic results of B-ultrasound with the pathological results of postoperative paraffin sections,the results of the comparative analysis showed that:paraffin sections were diagnosed as malignant tumors and B-modes were diagnosed as benign in 20cases;paraffin sections were diagnosed as benign tumors and B-mode diagnosis as 77 cases are malignant.The sensitivity was 80.39%,the specificity was 68.18%,the accuracy was71.80%,the positive predictive value was 51.57%,and the negative predictive value was89.18%.6.The preoperative MRI diagnosis results were compared with the postoperative paraffin section pathological results,and the results were as follows:10 cases were diagnosed as malignant tumors based on the pathological results and MRI was diagnosed as benign tumors,the pathological results were diagnosed as benign tumors and MRI diagnosis 40 cases were malignant tumors.Its sensitivity was 90.20%,specificity was 83.47%,accuracy was 85.47%,positive predictive value was 69.70%,and negative predictive value was 95.28%.7.Analysis of ultrasound staging and pathological diagnosis results(column connection number=0.543,P<0.001),the results showed that ultrasound staging and pathological diagnosis results have a high correlation8.MRI stage and pathological diagnosis results were analyzed(column connection number=0.609,P<0.001),the results showed that MRI stage and pathological diagnosis results have a high correlationConclusion 1.Patients with preoperative serum CA125,HE4,and ROMA can diagnose malignant ovarian tumors.All three can show a high diagnostic accuracy.ROMA has obvious advantages in the diagnosis of ovarian malignant tumors.2.B-ultrasonic combined with ROMA can combine the advantages of the two to improve the accuracy of benign and malignant diagnosis of ovarian tumors,and its application value can be used for clinical reference3.MRI examination can distinguish ovarian tumors very well,combined with ROMA for diagnosis,can effectively improve the accuracy of malignant results of ovarian tumors,and can be used in clinical applications...
