Objective With surgical-pathology results as the gold standard,By studying the value of MRI in diagnosing ovarian tumors and analyzing the misdiagnosis to provide reliable diagnostic basis for clinical.Materials and Methods Collecting suspected ovarian tumor in 120 cases in the First Affiliated Hospital of Chongqing Medical University, all of them were performed MRI before laparoscopy or laparotomy Analysising of the image features and misdiagnosis.Meanwhile, The MRI staging were Compared with postoperative pathological staging results.Results 1. The MRI diagnostic accordance rate of ovarian tumors was 88.3%(106/120) and the disdiagnostic rate was 11.7%(14/120); 2.Ovarian cyst,endometriosis cyst,ovarian teratoma,ovarian cystadenoma and Ovarian malignant tumor have their own MRI features. High signal can be fat-suppressed to low signal by fat suppression sequence which was helpful to diagnose teratoma with the high signal of endometriosis cyst and hemorrhagic cysts; 3.There were significant differences in the tumor boundary clear or not, wall nodules, surrounding tissue invasion, peritoneum and omental involvement, pelvic lymph node, DWI signal,with or without enhancement and ascite between the 78 cases of benign and 26 cases of malignant tumors. Otherwise,there were no difference in ovarian unilateral or bilateral involvement;4. The qualitative accuracy of ovarian malignant tumors was 100%. MRI staging showed that 4 cases were stage â… ,6 stage â…¡,12 stage â…¢ and 3 stage IV. While pathological staging results were 3 cases stage â… ,5 stage â…¡,14 stage â…¢and 3 stage â…£.MRI staging accuracy was 80%(20/25), but there was some difficulty in diagnosing cell types.Conclusion 1. MRI has an important role in diagnosing common ovarian tumor and the accuracy is very high;2. Common ovarian tumor have their own MRI features,MRI-STIR can distinguish bleeding from fat which has great value in the differential diagnosis between teratoma and hemorrhage cyst;3. MRI can distinguish malignancies from benign tumours according to ovarian tumor boundary clear or not, any wall nodule, surrounding tissue invasion, peritoneal and omental presence of metastasis, lymph node enlargement, DWI signal,with or without enhancement and ascite;4.MRI has a high accuracy in diagnosing common ovarian tumors. And its staging has good coinsistance with pathological staging. Preoperative MRI examination was of great importance for clinical assessment operation difficulty and prognosis. |
