ObjectiveTo analyze the influencing factors of breast-feeding from different angles,to construct the continuous nursing mode,to carry out the continuous nursing reasonably,to explore the influence of the continuous nursing on the self-care ability of primipar a and to evaluate the effect of the model on the breastfeeding rate,and to provide scientific basis for the clinical medical staff to formulate more effective comprehensive intervention strategy of breast-feeding.Methodsthis study belongs to the randomized intervention trial study.From the beginning of January 2019 to the end of December 2019,120 first-born women were admitted to a third-class hospital in Nanchang City.According to the random number table me thod,it was divided into routine group and intervention group,60 cases in each group,among which routine nursing was carried out in routine group.On the basis of routine group,the intervention group carried out comprehensive intervention on primipara by continuous nursing mode.Collect general data on the admission of primipara,fill out self-assessment form of primipara,evaluation form of health education satisfaction of breast-feeding during hospitalization and count the rate of exclusive breast-feeding and milk deposition within 48 h.Follow-up after discharge statistics 6 weeks,4 months,6 months exclusive breast-feeding rate,maternal self-care self-assessment scale score,milk deposition rate assessment of the implementation of continuous care on exclusive breast-feeding rate,maternal self-care ability and breast patency.all the data were analyzed by SPSS22.0 statistical software.the main statistical methods included descriptive analysis,independent sample t test,duplicate measurement of contrast,χ2 test and so on.Result1.Between the two groups,the exclusive breastfeeding rates were 48.1%,88.9%,92.6% and 81.5% in the intervention group and 37.0%,74.1%,68.5% and 40.7% in the routine group.There was no significant difference in exclusive breastfeeding rates between the intervention and routine groups 48 h postpartum(P >0.05).exclusive breastfeeding rates in the intervention group were higher at 6 weeks,4 months and 6 months postpartum than in the routine group(P <0.05).2.The results showed that the scores of self-care self-assessment scale were hi gher in the intervention group than in the routine group(P <0.01)in 48 h 、6 weeks,4 months and 6 months,and the scores of self-care self-assessment scale in48 weeks,4 months and 6 months.The three dimensions of self-care attitude,self-care knowledge and self-care skills in the self-assessment scale of maternal self-care in the intervention group were higher than those in the routine group(P <0.05)in 48h、6 weeks,4 months and 6 months after delivery.3.Comparison of milk silting rates between the two groups showed that the milk silting rates in the intervention group were 3.7%,7.41%,3.7% and 5.56% at 48h、6 weeks,4 months and 6 months after delivery,and 5.56% in the routine group.The incidence of milk deposition in the two groups was not statistically significanth 48 postpartum(P >0.05).and the incidence of milk deposition in the intervention group was lower than that in the routine group(P <0.05)at 6 weeks,4 months and 6 months postpartum.4.The results showed that the total score of breast-feeding health education satisfaction in the intervention group(38.15 ± 7.49)and the routine group(31.44 ±5.61).Health education satisfaction score of breast-feeding during hospitalization in intervention group was significantly higher than that in routine group(P <0.01).Conclusions1.Through the guidance of breast-feeding knowledge education,the correct nursing posture guidance for primipara during delivery and hospitalization,the establishment of breast-feeding support system after discharge,and the implementation of continuous nursing mode,the exclusive breastfeeding rate has been further improved,which provides scientific basis for clinical medical staff to formulate more effective comprehensive intervention strategies for breast-feeding.2.The comprehensive intervention of continuous nursing mode in primipara is helpful to improve the ability of self-care of primipara,which is worthy of clinical promotion.
