Object:To explore the safety of painless colonoscopy and the detection of colorectal disease in obese patients.Methods: A total of 292 patients who underwent colonoscopy at the Digestive Endoscopy Center of our hospital from October 2018 to July 2019 were included.they were divided into obese common group(G1),obese painless group(G2),normal group(G3)and normal painless group(G4),according to body mass index(BMI in the range of 18.5-23.9kg/m2 is abbreviated as normal,and BMI?28 kg/m2 is abbreviated as obesity)and method of examintion(common colonoscopy referred to as common,painless colonoscopy referred to as painless).The heart rate(HR),mean arterial pressure(MAP),blood oxygen saturation(SPO2),possible adverse reactions and detection of colorectal disease in the four groups of patients were observed and recorded in detail before the colonoscopy(t1),when they reach the ileocecal section(t2),and at the end of the test(t3).Results:The MAP and HR at t2 at the time of enteral examination in the painless group were significantly lower than those at t1(P <0.05).The SPO2 at G2 at t2 was significantly lower than that at t1(P <0.05).There was no significant difference in MAP,HR,and SPO2 at t2 and t3 in patients undergoing common colonoscopy compared with that at t1(P> 0.05).The incidence of adverse reactions was higher in the general colonoscopy group during the examination(P < 0.001).The time of reaching to the ileocele of the general colonoscopy group was significantly prolonged compared to the painless colonoscopy group(P<0.05),but there was no significant difference in the total time to complete(P> 0.05).The successful rate of G3 colonoscopy in the general colonoscopy group was significantly higher than that of G1(P <0.05);The total detection rate of colorectal diseases was 70.9%,The detection rates of colorectal polyps,colorectal inflammation,hemorrhoids,colitis,large intestinal diverticulum and submucosal lesions from high to low was respectively 44.2%?11.0%?4.1%?3.4%?3.1% and 2.4%,of which the detection rate of colorectal adenoma was 22.3%.The detection rate of colorectal disease in male patients was significantly different than that in female patients(P <0.05),the detection rate of colorectal disease in patients with different body mass index was statistically significant(P <0.05).Conclusion: Compared with ordinary colonoscopy,obese patients undergoing painless colonoscopy have fewer adverse reactions,higher postoperative satisfaction,and higher disease detection rates.After strictly grasping the indications and contraindications for anesthesia,the obese patients within a certain range undergo painless colonoscopy as safe and comfortable as ordinary colonoscopy.
