Background:It is still controversial whether arthroscopic acromioplasty and subacromial decompression are advantageous to postoperative recovery of patients undergoing arthroscopic rotator cuff repair. This study was based on the analysis of the efficacy of rotator cuff repair with or without acromionplasty or subacromial decompression in patients with rotator cuff injury, and further review was performed based on the postoperative efficacy. Methods:Studies meeting the inclusion criteria were screened and included from systematic literature search for electronic databases including Medline,Embase,CINAHL and Cochrane library was conducted from 2000 and 2020.A total of 10 comparative studies, including 7 randomized clinical trials(RCTs)study,2 cohort studies(CS),1 prospective studies, were screened to meet the inclusion criteria.By comparing the collected postoperative data :UCLA score,Constant score,ASES score,SST score,VAS score, shoulder joint range of motion(ROM)and postoperative Reoperation rate, the postoperative recovery of shoulder joint under different surgical methods was analyzed. Results: All postoperative data of 781 patients(the numbers with arthroscopic acromioplasty or subacromial decompression / the numbers without arthroscopic acromioplasty or subacromial decompression:397/384)were collected. Our study demonstrated the UCLA score,Constant score,ASES score,SST score,VAS score and the range of motion of shoulder joint(ROM)when rotator cuff repair was performed under arthroscopy, accompanied with or without acromioplasty or subacromial decompression. No statistical differences were analyzed in the data of short-term follow-up over a 2-5 year period. The reoperation rate(12.7%)of the group without acromioplasty or subacromial decompression was higher than that of the group with acromioplasty or subacromial decompression(8.2%),but there was no significant statistical significance between the two groups. Conclusion:On the basis of the currently available evidence, there was no statistically significant difference in clinical outcomes: Constant score,ASES score,UCLA score,SST score,VAS score and the range of motion of shoulder joint(ROM)for patients undergoing arthroscopic rotator cuff repair with or without acromioplasty or subacromial decompression at short-term follow-up.more preferable data resources are needed to further determine whether there is any difference between the two groups in the postoperative recovery effect of patients in the medium and long term or other postoperative efficacy. Finally, both the short-term efficacy under rotator cuff repair shows similar outcomes. It is worthwhile for the surgeons to consider the method of operation according to the specific situation of the patient and the rotator cuff tear.
