Clinical Observation On Acute Gouty Arthritis Of Knee Joint Treated With Weizhong Blood-letting Puncture And Cupping Therapy Combined With Conventional Drugs

Objective:Observation of the clinical efficacy of Weizhong puncture and cupping therapy combined with conventional drugs in the treatment of acute gouty arthritis(AGA)of the knee joint has laid the foundation for the further clinical application of this method.Method:Sixty AGA patients were recruited by the inclusion and exclusion criteria,and divided into control group and test group by the method of random number table,with 30 cases for each group.The control group were given colchicine by oral medication(1 mg for the first dose,0.5 mg every 1 hour whereafter,1 mg per day for the second and third day);The test group were given Weizhong puncture and cupping therapy one time based on the treatment of the control group.The two groups of patients,when at VAS≥4,were given Diclofenac Sodium Sustained-Release Capsule(50 mg each time,twice a day).Prior to and after treatment for 1,3,and 5 days,clinical effects by the two therapeutic schedules were evaluated by means of visual analogue scale(VAS),symptoms and signs scale,statistics of analgesic dose,sera diagnosis of CRP,ESR,WBC count and calculation of clinical efficiency etc.Result:1.Resting and motional VAS: For comparison within the group,the resting and motional VAS after 1,3,and 5-day treatment were all lower than that prior to the treatment,with statistical significance in the differences(P<0.01).For comparison among groups,the test group revealed a lower level of resting and motional VAS than that of the control group after 1,3,and 5-day treatment,with statistical significance in the differences(P<0.01).This verifies the efficiency of the pricking and cupping method in easing arthralgia under resting and motional conditions.2.Symptoms and signs scale: For comparison within the group,the symptoms and signs scale after 1,3,and 5-day treatment were all lower than that prior to the treatment,indicating efficacy of the two therapeutic schedules in easing arthralgia,red and swollen,tenderness,and movement disorder etc.For comparison among groups,after the 1-day treatment,the test group showed a lower level of arthralgia and the total scale than that of the control group;after the 3-day treatment,the test group showed a lower level of arthralgia,red and swollen,tenderness,movement disorder,and the total scale than that of the control group;and after the5-day treatment,the test group showed a lower level of arthralgia,red and swollen,tenderness,movement disorder,and the total scale than that of the control group.This indicates the efficiency of the pricking and cupping method in easing arthralgia,and patients’ symptoms and signs get effective improvement on the 5th day.3.Per capita integral dose of Diclofenac Sodium Sustained-Release Capsule by oral medication: during the treatment,the test group patients had lower per capita integral dose of Diclofenac Sodium Sustained-Release Capsule by oral medication than that of the control group.This reveals that the combined therapy with pricking and cupping method can ease arthralgia and reduce the analgesic dose for AGA patients.4.Lab index: For comparison within the group,both groups showed a lower level of serum CRP,ESR,and WBC count after 1,3,and 5-day treatment.This indicates the efficiency of the two therapeutic schedules in eliminating related inflammations.For comparison among groups,the test group showed a lower level of serum CRP,ESR,and WBC count after 1,3,and 5-day treatment that that of the control group,indicating the efficiency of the combined therapy with pricking and cupping method in eliminating related inflammations and easing AGA patients’ arthralgia,red and swollen,and heat pain.5.Clinical effect analysis: The study found that the total efficiency of the test group was93.33%,superior to the control group with 83.33%.This verifies the clinical effect of the combined therapy with pricking and cupping method on AGA.6.security indexes:The test group and control group were not present adverse reactions during treatment.Conclusion:Weizhong puncture and cupping method combined with conventional drugs has significant efficacy in eliminating AGA patients’ arthralgia,red and swollen,disorder of joint motion,and reducing serum CRP,ESR,WBC count and inflammatory reactions.The efficacy was superior to the control group.This method is safe with little adverse effect,and has great potential in clinical application.
