Study On The Correlation Between Different TCM Syndromes Of Type 2 Diabetes And Blood Uric Acid

Objective:Analysis of distribution of blood uric acid to figure out the correlation between serum uric acid TCM syndromes in patients with type 2 diabetes,and the correlation between blood uric acid metabolism and glucose metabolism.Methods:Choose type 2 diabetes patients who had admitted during the period from June to December in 2013 at Department of endocrinology of Zhejiang Province Traditional Chinese Medical Hospital.According to the Chinese Medicine 5 syndromes divided into 5 groups,including Yin deficiency and heat accumulation group,damp heat in spleen group,both qi and yin deficiency group,yin and yang deficiency group,blood stasis of channel group.Each group include of 30 people,as total it is 150 people.Collect the first hospitalization data.As the same time collect 30 normal people as control subjects.Base on this 150 people,divided into two groups according to the diagnostic criteria for hyperuricemia,which is type 2 diabetes mellitus complicated with hyperuricemia group and type 2 diabetes patients without hyperuricemia group.Evaluation of the relationships between serum uric acid and glucose metabolism,so as lipid metabolism and renal function,and correlation between serum uric acid and TCM Syndromes of type 2 diabetes,and the relationship between serum uric acid and related complications.Results:① All the type 2 diabetic patients 150 with different TCM syndrome after the statistical analysis showed that the age and gender had no statistical difference(P>0.05).Then divided these 150 patients into two group of normal SUA group and HUA group by serum uric acid level,basic by men and postmenopausal women>420μmol/L,pre menopausal women>360μmol/L.Results showed that SUA group include 19 cases,no significant difference between the high SUA group and normal SUA group of age and sex(P>0.05),can be comparable.②The comparison of SUA in type 2 diabetes showed that blood stasis of channel group>yin and yang deficiency group>both qi and yin deficiency group>control group>Yin deficiency and heat accumulation group>damp heat in spleen group.And blood stasis of channel group compared with the other 4 groups and the control group showed the statistical significance(P<0.05).Compared high SUA group with normal SUA level showed the high SUA group>normal SUA group,had statistical significance(P<0.05).③ Blood stasis of channel group of type 2 diabetes shows that there is the correlation between UA and FINS、HOMA-IR(P<0.05);The type 2 diabetic patients with high SUA group shows there is the correlation between UA and FINS,HOMA-IR,HbAlc,Cr(P<0.05).④The HUA group and the normal SUA group’s diabetic vascular disease complication rates respectively were 26.3%and 57.9%,with statistical significance(P=0.019);The HUA groups and the normal SUA group’s microvascular complication rate respectively were 68.4%and 42.7%,with statistical significance compared with normal SUA group(P=0.032);The HUA groups and the normal SUA group’s peripheral neuropathy complication rate was 47.4%and 24.4%,with statistical significance(P=0.038);The HUA group and the normal SUA groups hypertension incidence rate respectively were 68.4%and 41.2%,with statistical significance(P=0.024).Conclusion:①The SUA level in the blood stasis of channel in patients with type 2 diabetes was significantly higher,suggesting that can be used as an objective index of type 2 diabetes blood stasis of channel group typing context.②Type 2 diabetes blood stasis of channel group,there is correlation between SUA and FINS、HOMA-IR;Type 2 diabetic patients with HUA group,there is correlation between SUA and FINS,HOMA-IR,HbA1c,Cr.All of these suggested that control the level of SUA has certain significance for blood glucose and IR in type 2 diabetes patients.③The SUA level in type 2 diabetes mellitus has certain relevance with diabetic vascular disease complication,diabetic microvascular,peripheral neuropathy,and hypertension complication,suggested increased SUA level may indicate the type 2 diabetic patients with higher the risk of diabetic vascular disease,diabetic microangiopathy,peripheral neuropathy and hypertension.
