| Objective:Alcoh olic cirrhosis is a chronic,aggressive and diffuse liver disease caused by long-term heavy drinking,which has a long course of disease,a difficult disease,a poor prognosis and a serious impact on the quality of life of patients.At present,modern medicine is not satisfied with the therapeutic effect of alcoholic cirrhosis,and the side effects are extensive.Alcohol consumption in developing countries has increased year by year,leading to an increase in the incidence and mortality of alcoholic cirrhosis.Therefore,it is imperative to develop new targets and new therapies to prevent and control alcoholic cirrhosis.Combining with the results of modern m edical research and his own years of clinical observation and practice,practitioners developed the "Yiqi Jiedu Tongluo Method" under the guidance of TCM syndrome theory,and through clinical observation,this method and thinking are in line with the pathogenesis of chronic liver disease "QI deficiency and collaterals closure",and fully take into account the pathological characteristics of "toxic hepatic collaterals " in chronic liver disease,and the effect of reversing hepatic fibrosis is accurate.The purpose of this project is to observe the clinical effect of Yiqi Jiedu Tongluo Fang in the treatment of alcoholic cirrhosisMethod:T he research object of this topic is from th e first clinical Medical College of Beijing University of Traditional Chinese medicine hepatitis outpatient patients,according to the standard,screening in line with the group of patients a total of 32 cases,the use of a single group of their own control,32 patients are taking Yiqi Jiedu Tongluo treatment for 6 months,and with the addition an d reduction of disease,record,observe,The changes of TCM symptoms,signs,serological indexes,liver ultrasound integral of the subjects before and after treatment were compared and analyzed in stages according to the condition.The statistical software of SPSS20.0 is applied to the statistical analysis and processing of data.Results:In this study,36 patients with alcoholic cirrhosis who met the selection criteri a were observed,1 cases were shedding,3 cases were eliminated,and a total of 32 p atients were finally completed.It was found that the incidence of major drinking and alcoholic cirrhosis was dominated by men,the peak of the disease was around 55 yea rs old,the drinking history was about 20 years,and the daily drinking volume was th e highest in 80g/d,and the patients irn the group were mainly patients with decompen sated period(up to 78.12%).From the point of view of overall curative effect observat ion:① After the treatment of patients burnout weakness,epigastric abdominal distensio n full,irritable irritability,less food,rib pain,mouth bitter,urine yellow,insomnia or multi-dream,defecation is not clear,dark complexion than before treatment significant)y reduced(p<0.01),skin itching,body yellow than before treatment improved(p<0.05),hand and foot numbness,There was no significant improvement in red filaments than before treatment(p>0.05).② Liver function Index ALT,AST,GGT,ALP,TBIL,ALB after 6 months of treatment than before treatment has a significant improvement(p<0.01),indicating that the treatment can effectively improve liver function.③ Coagulation function Index PT,PTA treatment after 6 months have a significant improvement(p<0.01),indicating that treatment can effectively improve the coagulation function.④PLT and MCV were improved before treatment(p<0.01).⑤ There was no significant differe nee in ultrasound liver morphological integral before and after treatment(p>0.05),sug gesting that Yiqi Jiedu Tongluo therapy did not significantly improve the liver morpho logy in the short term.(2)From the point of view of phased curative effect observation:The patients with alcoholic cirrhosis compensation period,①The indexes of AST,ALP,TBIL and ALB were significantly improved(p<0.01)before and after treatment an d the improvement of ALT and GGT(p<0.05)was explained,which indicated that the treatment could effectively improve the liver function during the compensatory period.②There were improvements in PT and PTA before and after treatment(p<0.01),whi ch could improve the compensatory period coagulation function.③After treatment,PL T and MCV were significantly improved before treatment(p<0.01).④There was no sig nificant change in liver morphological integral under 6 months of treatment,which in dicated that there was no significant improvement in liver morphology in patients with compensatory period in short time.Patients with decompensated period of alcoholic c irrhosis ①The indexes of ALT,AST,GGT,ALP,TBIL and ALB were significantly im proved(p<0.01)before and after treatment,which showed that the treatment could effe ctively improve the liver function of patients with decompensated period.②There wer e improvements in PT and PTA before and after treatment(p<0.01),suggesting that th is party could improve coagulation function in patients with decompensated period.③There were improvements in PLT and MCV before and after treatment(p<0.01).④ T here was no significant difference in liver morphological integral before and after treat ment(p>0.05),suggesting that the treatment of liver morphology in patients with deco mpensated cirrhosis was not significantly improved in the short term.(4)The main adv erse events:individual patients with double lower limb weakness,the number of stool increase phenomenon,after self-relief,no other obvious symptoms of discomfort.Conclusion:Yiqi Jiedu Tongluo Fang has obvious improvement of symptoms and signs,liver retention enzyme,indicating that Yiqi Jiedu Tongluo Fang is safe and effective in the treatment of alcoholic cirrhosis,the economic burden is less than Western medicine,fully shows that combination of disease differentiation and syndrome differentiation in traditional Chinese medicine treatment of individual pathogenesis,so Yiqi Jiedu Tongluo Fang is worthy of further study and observation. |
