| Objective:To master the prevalence and characteristics of cysticercosis in Dali Prefecture,Yunnan Province,and to analyze the health-related quality of life and its influencing factors in patients with cysticercosis,and to analyze the disability weights of cysticercosis based on subjective and objective evaluation methods.Methods:The first part is the analysis of the epidemiological characteristics of new hospitalized cases of cysticercosis in Dali.All the cysticercus cases in Dali parasitic diseases control center from January 1,2014 to December 31,2017 were collected during this retrospective study.According to the retrospective questionnaire of hospitalized cases of cysticercosis,cases meeting the diagnostic criteria were included in the analysis.Basic demographic information and epidemiological characteristics of new hospitalized cases from 2014 to 2017 were described and analyzed,and ArcGIS software was used to describe their distribution in Dali prefecture.The second part is the health-related quality of life study of cysticercosis.Dali parasitic diseases control center was selected as the investigation point,the socio-demographic characteristics and clinical symptoms of hospitalized patients with cysticercosis were investigated by using a unified questionnaire and face-to-face interview.EQ-5D+C was used to evaluate the health-related quality of life of patients with cysticercosis,and multiple linear regression analysis was used to explore the influencing factors of the health-related quality of life of patients.Meanwhile,the Disability Weight(DW)of NCC was obtained based on EQ-VAS score and symptom outcome.Results:In the first part,a total of 1,552 hospitalized cases of cysticercosis were collected,including 549 new hospitalized cases.Patients with neurocysticercosis(NCC)were the majority of the new hospitalized cases,accounting for 91.26%(501/549).The basic demographic characteristics were 333 males(60.66%)and 216 females(39.34%),the average age was 37.86±10.35years old.The main ethnic group was Bai(55.01%302),and the main occupation distribution was farmers(75.59%415).In terms of epidemic distribution,Dali Prefecture has new cases in all counties(cities),but the number of cases in each county(city)is different,among which Dali City and Eryuan County are the main ones,accounting for 26.78%(145/549)and 19.49(107/549),respectively.In the second part,a total of 320 questionnaires were collected.After excluding those that did not meet the inclusion criteria,the remaining 270 questionnaires were valid.The main clinical symptom of the first case was headache(76.19%),and the main clinical symptom of the return case was memory loss(67.46%).In terms of six health dimensions,cognitive ability has the largest proportion(71.90%).Analysis of influencing factors indicates that occupation is a protective factor for daily activities(OR=0.39),and marital status is a risk factor for pain and discomfort.(OR = 2.27),age is a risk factor for cognitive difficulties.In terms of health-related quality of life,the average score of EQ-VAS of NCC patients was 69.97±13.03,and there was no significant difference between the first-visit and re-treated patients(t=0.63,P>0.05).Multivariate analysis found that daily activities,pain/discomfort,anxiety/depression and cognitive ability were the influencing factors of health-related quality of life(P<0.05),among which cognitive ability had the largest impact(standardized regression coefficient was-0.332).In terms of DW,DWi0.30(0.20-0.40)based on EQ-VAS score,0.30(0.20-0.40)and 0.30(0.20-0.40)between the first-visit and re-treated patients,respectively,there was no statistically significant difference(Z=0.74,P>0.05).There was no significant differences between the sexes(Male 0.30(0.20-0.40)/Female 0.30(0.20-0,40),there was no statistically significant difference(Z=-0.15,P>0.055).DWi values between age groups were not completely equal(χ2=13.25,P<0.05),which were 0.20(0.10-0.20)(5-14 years old),0.30(0.20-0.40)(15-29 years old),0.30(0.20-0.40)(30-44 years old),0.30(0.20-0.40)(45-59 years old),and 0.30(0.30-0.40)(≥60years old),respectively,there was statistically significant difference.The DW2 obtained based on the symptom outcome was 0.14(0.07-0.35),and the first-visit and re-treated patients were0.14(0.07-0.35)and 0.08’(0.03-0.34),respectively,and the difference was not statistically significant(Z=3.01,P<0.01).The combined male and female DW2 were 0.13(0.05-0.34)and 0.13(0.03-0.35),respectively,and the difference was not statistically significant(Z=0.19,P>0.05).DW2 values between age groups were not completely equal,which were 0.03(0.03-0.11)(5-14 years old)、0.13(0.05-0.16)(15-29 years old)、0.14(0.05-0.35)(30-44 years old)、0.14(0.05-0.35)(45-59 years old)、0.05(0.02-0.16)(≥60years old),respectively.The difference was statistically significant.Conclusion:The new hospitalized cases of cysticercosis in Dali are mainly Bai nationality,male and farmers aged 30-44 years.The new hospitalized cases of cysticercosis are distributed throughout the prefecture,and are mainly concentrated in Eryuan and Dali.The health-related quality of life of patients with cysticercosis is severely impaired.Cognitive ability are prominent health problems of patients with NCC.Based on the subjective EQ-VAS score and the DW value calculated from the symptom outcome,it is suggested that the evaluation of the quality of life of NCC patients needs to be combined with subjective and objective evaluation methods in order to comprehensively judge the impact. |
