| Background:Chronic obstructive pulmonary disease(COPD)patients are often accompanied by sleep disorders,which not only affect the quality of sleep in patients but also affect the daytime function and mental status of patients.Sleep disorders also cause cognitive dysfunction,decreased immune function,and reduced exercise endurance.It seriously affects the quality of life and social function of patients.At present,there is no clinical treatment method for COPD with sleep disorder.Commonly used sedative sleeping pills have a fast effect but with many side effects as well.Massage is one of the characteristic treatments of traditional Chinese medicine,and it is economical,simple,and with fewer side effects.Therefore,we aim to explore the clinical efficacy of massage therapy on COPD combined with sleep disorder to provide evidence for patients with COPD and sleep disorder seeking simple and effective rehabilitation physiotherapy methods.Objective:To observe the clinical effect of massage therapy on sleep disturbance in COPD patients.Methods:A randomized,controlled,and exploratory study was used.From January 2019 to March 2019,patients from the China-Japan Friendly Hospital and the Third Affiliated Hospital of Beijing University of Chinese Medicine who met the inclusion criteria were randomly divided into control group and treatment group.For the control group,patients were given routine treatment and nursing care for COPD while the treatment group was further incorporated with massage therapy.The massage procedure mainly involved the head and back.The focus was placed on the Du and Foot-Taiyang meridians,Yintang,Cuanzhu,Taiyang,Baihui and Jiaji acupoints.Each massage treatment lasted 20 minutes,for a total of 7 days.Both groups were assessed using the Pittsburgh Sleep Quality Index(PSQI),Insomnia Severity Index(ISI),COPD Assessment Test(CAT),Modified Medical Research Council(mMRC),Self-rating Depression Scale(SDS),and Self-rating Anxiety Scale(SAS)before and after treatment to observe the changes in the quality of sleep and quality of life of the patients.Finally,the efficacy of the treatment for insomnia was evaluated based on the criteria set out in the "Guidelines for Clinical Research of New Chinese Medicine".Statistical analysis was conducted using SPSS 20.Results:35 patients were included in this study(17 cases with 2 drop-outs for the treatment group and 18 cases with 3 drop-outs for the control group).There was no significant difference in the baseline data between the two groups before treatment(P>0.05),and the two groups were comparable.After treatment,the ISI,SDS,SAS,and CAT scores were all lower than before treatment,and the difference was statistically significant(P<0.05),but the difference between the two groups was not statistically significant(P>0.05).After treatment,the mMRC score decreased significantly in the treatment group(P<0.05),and the treatment group had a statistically lower score than the control group(P<0.05).After treatment,the PSQI total score,sleep quality,sleep time,and sleep efficiency of the treatment group decreased significantly(P<0.05).No significant change was observed in the PSQI total and individual factors scores in the control group(P>0.05).No statistical difference was observed between the PSQI scores of the two groups(P>0.05).The total effective rates were 73%for the treatment group and 40%for the control group,and no statistical difference was observed between the two groups(P>0.05).Conclusion:①Routine therapy combined with massage therapy can effectively improve the quality of sleep and life of patients with COPD.②Massage can improve breathing difficulty.③No significant change in treatment efficacy was observed upon the incorporation of massage therapy. |
