Professor Yao Naili's Experience In Treating Functional Dyspepsia And Clinical Observation Of Jianpi Zhuyun Decoction

Aim:To sum up the experience in treating functional dyspepsia(FD)of Professor Naili Yao,and to verify the changes of clinical symptoms,quality of life,mental state and recurrence rate of patients with functional dyspepsia by observing and evaluating Jian Pi Zhu Yun Decoction,and verifying the effectiveness and safety.Methods:The ancient and modern literature related to FD was studied and sorted out,and the clinical experience of professor Naili Yao in the diagnosis and treatment of FD was systematically sorted out through the consultation with the doctor.A prospective self-controlled clinical study method was used to select 75 patients with FD who were admitted to the gastroenterology department of Guang’an men Hospital of China Academy of Chinese Medical Sciences from November 2018 to January 2019.The patients of FD enrolled in the treatment of Jian Pi Zhu Yun Decoction,the medicine was uniformly boiled by the Guang’an men Hospital decocting room,one dose per day,each dose of 400ml,divided into early and late on an empty stomach.One week is a course of treatment for a total of 4 weeks.The patient’s safety index was recorded once every 0 and 4 weeks,and the TCM syndrome score,HAMA,HAMD,FDDQL scale scores were recorded for each observation after 0,4 weeks and 1 month follow-up.The difference in efficacy before treatment,after treatment,and after follow-up was compared,and the recurrence rate was counted.All data were statistically analyzed using SPSS 25.0 software.Result:This paper summarizes professor Naili Yao’s clinical experience in the treatment of FD.In terms of etiology,professor Naili Yao believes that the etiology of this disease is complex,and the pathogenic factors are diverse,but ultimately the injury to the spleen and stomach is the root,and the spleen and stomach transport is unfavorable,resulting in the occurrence of the disease.In terms of pathogenesis,professor Naili Yao believes that"weak spleen and stomach,abnormal lifting,unfavorable transportation and internal stagnation of dampness and turbidity,are the basic pathogenesis of FD.In terms of diagnosis and treatment,professor Naili Yao believes that the treatment of this disease should strengthen the spleen and stomach to strengthen its fundamental,paying attention to adjust the rise and fall of the middle coke air machine,to facilitate the spleen and stomach transport,and treatment does not forget the overall recuperation,in order to prevent the recurrence of the disease.In the aspect of differentiation and treatment,professor Naili Yao believes that we should grasp the basic pathogenesis of the disease,combine the disease and syndrome,control the overall situation,use drugs flexibly and give consideration to multiple diseases.In the clinical study of Jian Pi Zhu Yun decoction in the treatment of FD,5cases caused by loss of follow-up and failure to take medicine as required were excluded,and a total of70 cases under clinical observation were actually completed.1.TCM syndrome efficacy evaluation:after 4 weeks of treatment,35 cases(50.00%)of the patients were clinically cured of the main symptoms and signs,18 cases(25.71%)of the obvious effects,12 cases(17.14%)of the effective effects,5 cases(7.14%)of the ineffective effects,65 cases of the total effective cases,and the total effective rate was92.86%2.The quality of life assessment:at 4 weeks after treatment,the patients’ daily life,care,diet,sleep,feeling unwell,health,disease control,the pressure,the eight single points,as the extension of treatment time,with varying degrees of increase,the integral is proportional to the quality of life,integral is higher,the better the quality of life,that Jian Pi Zhu Yun decoction in improving the patients survival quality evaluation indicators have curative effect.The difference between before and after treatment was statistically significant((P<0.01).3.Efficacy evaluation of anxiety and depression:after 4 weeks of treatment,the scores of anxiety and depression were reduced to different degrees.The lower the scores were,the better the anxiety and depression of the patients were.The difference was statistically significant(P<0.01).4.Evaluation of recurrence rate:after 4 weeks of treatment,patients were discontinued and followed up for 1 month.There was 1 case of severe recurrence(1.43%),2 cases of general recurrence(2.86%),2 cases of mild recurrence(2.86%),and 65 cases of non-recurrence(92.86%).5.Safety evaluation:during the clinical study,no adverse reactions occurred in the 70 patients who completed clinical observation.General physical examination items including blood pressure,respiration,body temperature and pulse were not abnormal,blood routine,urine routine,stool routine,occult blood,liver and kidney function examination,electrocardiogram and electrolyte were not obvious abnormalities before and after treatment.Conclusion:Professor Naili Yao has a unique understanding of the etiology,pathogenesis,diagnosis and treatment of FD.The therapeutic effect of Jian Pi Zhu Yun decoction in the treatment of FD is positive,which can improve the clinical symptoms,quality of life,anxiety and depression,and reduce the recurrence rate of FD to some extent.
