Objective:To summarize Professor Yao Naili's understanding of pathogenesis and clinical experience of gastroesophageal reflux disease(GERD).Through the statistical analysis of the syndrome elements of GERD patients,this paper summarizes Professor Yao's understanding of the pathogenesis of GERD;at the same time,it observes the clinical effect of Jia Wei Qi Xian decoction on GERD patients in order to provides reference for clinical diagnosis and treatment of GERD.Methods:By consulting and sorting out the relevant literature of GERD,and combining the knowledge learned from the teachers in the outpatient department,this paper summed up Professor Yao's understanding of the pathogenesis of GERD and his experience in diagnosis and treatment.Through cross-sectional study,the medical records of 120 GERD patients from August 2018 to December 2019 were collected.After diagnosis by Professor Yao and senior experts who are familiar with and inherit Professor Yao's academic thoughts,the inductive factors were extracted.Professor Yao and senior experts who are familiar with and inherit Professor Yao's academic thoughts were interviewed according to the research results,and summarized Professor Yao's view on pathogenesis of GERD.In the prospective self-control research,60 GERD patients were treated with Jia Wei Qi Xian decoction,one dose per day,200ml in the morning and 200ml in the evening on an empty stomach,for a total of 8 weeks.Before treatment,4,8 weeks of treatment,and 4 weeks of follow-up,the patient's Gerd Q scale and TCM syndrome score were observed and recorded;the safety indicators of the patients were recorded before observation and after 8 weeks of treatment.The difference of curative effect before and after treatment was compared,and the recurrence was observedResults:In terms of GERD's name,Professor Yao Naili advocates that it can be classified into "esophageal Bi" and "esophageal Dan" for diagnosis and treatment.In terms of pathogenesis,Professor Yao thinks GERD is caused by weakness of spleen and stomach,disharmony of liver,spleen and stomach,and disorder of qi.Qi stagnation and deficiency,phlegm and heat are important pathological factors.In terms of treatment,Professor Yao thinks that we should pay attention to harmonizing the functions of viscera and removing the evil qi,and treat comprehensively according to the specific situation of patients.There were main results of the cross-sectional study:the syndrome elements of GERD were mainly concentrated in spleen,liver and stomach;the distribution of qi deficiency,qi stagnation,phlegm,yin deficiency and Jin deficiency in Nerd and RE is different(P<0.05).Among patients with GERD,those with qi stagnation,dampness,phlegm,heat and yin deficiency were more likely to develop Nerd(P<0.05);those with qi deficiency were more likely to develop Re(P<0.05).There were main results of the cross-sectional study:the total and single score of TCM syndromes and the Gerd Q scale score of the patients decreased after 0,4,8 weeks of treatment(P<0.05).The total effective rate of TCM syndrome was 93.1%and the recurrence rate was 8.62%.During the clinical study,58 patients who completed clinical observation did not have any adverse reactions.Conclusions:Professor Yao Naili's believes that liver,spleen,and stomach disorders and qi disorders are the pathogenesis basis of GERD,qi stagnation,dampness,sputum,and heat are important pathogenic factors of NERD;qi deficiency is an important factor in the occurrence of RE.Jia Wei Qi Xian Decoction is effective in treating GERD and can be further promoted.
