Research On The Distribution Characteristics Of TCM Syndromes Of Rheumatoid Arthritis In Active Period And The Rule Of Medication

OBJECTIVE: To explore the characteristics of syndrome distribution and medication of active rheumatoid arthritis(RA)in order to provide more experience for the treatment of active RA based on syndrome differentiation.Materials and Methods: Based on the data of one-year inpatients of the Rheumatology Department of Affiliated Hospital of Liaoning University of Traditional Chinese Medicine(July 2017 to June 2018),the cases were screened in strict accordance with the admission and exclusion criteria.And the database was established by WPS Office form and statistical analysis was performed,using SPSS19.0 and SPSS Modeler 14.1 software for data mining,analysis of the distribution characteristics of RA to explore the rule of medication.Result:1.In this study,207 cases were collected,with a male to female ratio of 1:7.28 and an average age of 60.83±9.40 years.2.Distribution of syndromes(in proportion): damp-heat obstruction combined with blood stasis(22.22%)> phlegm-blood stasis(18.36%)> cold-dampness obstruction combined with blood stasis(17.39%)> cold-dampness obstruction(14.97%)> damp-heat obstruction(8.70%)> liver and kidney insufficiency combined with blood stasis(7.25%)> liver and kidney insufficiency(5.80%)> Qi-Yin Deficiency combined with blood stasis(2.42%)>Qi-Yin Deficiency(1.93%)> rheumatism obstruction syndrome.= Qi-blood deficiency syndrome(0.48%).3.94 prescriptions were collected in this study,involving 149 Chinese herbal medicines,1343 times in total,17 categories of drugs.Glycyrrhiza uralensis,Radix Paeoniae Rubra and other20 kinds of Chinese medicines with frequencies(> 20)are commonly used in the treatment of active RA,while tonifying deficiency,eliminating rheumatism,clearing away heat and promoting blood circulation and removing blood stasis are the main medicines in the treatment of active RA.4.Cluster analysis was carried out on 20 Chinese herbal medicines whose times of medication were more than 20.Six groups of cluster medicines were obtained:Class 1 Radix Aconiti Root,Rhizoma Atractylodis,Rhizoma Cinnamomi,Radix Astragali,Class 2 Glycyrrhizae,Class 3 Radix Rehmanniae,Poria cocos,Angelicae sinensis,Rhizoma Chuanxiong,Active Dilong,Radix Paeoniae Alba,Class 4 Achyranthes bidentatae,Class 5 Ramulus Atractylodis,Cortex Phellodend.5.A total of 106 association rules for drug pairs and drug groups were obtained by association analysis of 149 traditional Chinese medicines,among which the highest degree of support for Radix Paeoniae Rubra+Radix Notopterygii was 32.886%.Conclusion:1.Active RA is mainly based on empirical evidence,damp-heat obstruction and blood stasis syndrome is the most common,the prevention and treatment of "blood stasis" should be paid special attention.2.In the active period,RA is mainly used to dispel pathogenic factors,supplemented by strengthening the body,especially in dispelling wind and dampness,promoting blood circulation and removing blood stasis and clearing away heat.3.The law of medication is in accordance with the characteristics of syndrome distribution.Disease,syndrome differentiation and medication are closely related.
