Observation On The Curative Effect Of Buzhengxugu Pill Combined With External Fixator In The Treatment Of Intertrochanteric Fractures Of The Elderly With Liver And Kidney Deficiency

Purpose:To observe the clinical effect of Buzheng Xugu Pill and external fixator in the treatment of elderly femoral intertrochanteric fracture of liver and kidney deficiency typeMaterial and method:In this paper,through a retrospective analysis,selection of the affiliate hospital of Liaoning Traditional Chinese Medicine University of orthopedicas ward since June 2016-June 2018 were analyzed,Buzheng Xugu Pill and external fixator of surgical treatment of elderly patients with femoral intertrochanteric fracture.and 35 cases of AO type A1.1 type in 2 cases,4 cases were type A1.2,A1.3 type 3 cases;There were 9 cases of type A2.1,14 cases of type A2.2,and 3 cases of type A2.3.Age: 72-95 years old,average:85.9 years old.The average operative time,intraoperative blood loss,complications,VAS score,collodiaphysial angle,anterior obliquity,and weight-bearing collodiaphysial angle and anterior obliquity were observed.TCM Syndrome Scale(marked effect: fewer symptoms≥70%;effective: syndrome integral reduction ≥30%,< 70%;ineffective: not up to the above criteria).Harris scoring system(excellent > 90,good 80-89,good 70-79,poor < 70)was used for 6-8months postoperative follow-up to evaluate the treatment effect.Results:The average operation time of 35 patients was 34.2min(28-60min)and the average bleeding volume during surgery was 6.7ml(5-25ml).One patient died of a internal disease within 6 months of the operation,all the remaining 34 cases were followed-up for 6months,fracture union in follow-up patients,average clinical union time: 12w(10-14w).Three of the patients had superficial infections with external fixed threaded needles.Oral antibiotics and positive needle skin incision drainage routine replacement dressing with 2 weeks infection control.2 cases of fixed needle loosening,timely adjustment,no functional recovery in the later stage,1 case of joint stiffness,and 2 cases of coxa vara.There was not complications such as fracture of external fixation,nonunion,nerve vessel injury.VAS score(7.8±0.9 at admission,6.9±0.6 for 3 days after surgery,3.3±0.5 for 5 days after surgery,3.3±0.9 for 2 weeks after surgery,0.9±0.3 for 12 weeks after surgery,and 0.1±0.2for 6 months after surgery).Collodiaphysial angle(128.1±3.9 for 3 days after surgery,127.0±5.0 for 6 weeks after surgery,127.5±4.0 for 12 weeks after surgery,127.6±4.0 for 6months after surgery).Anterior obliquity(13.2±1.1 for 3 days after surgery,13.0±1.2 for 6weeks after surgery,13.0±1.1 for 12 weeks after surgery,12.9±1.0 for 6 months after surgery).Harris hip joint function score(34.1±3.3 for 6 weeks after surgery,53.7±3.6 for 12 weeks after surgery,86.9±6.2 for 6 months after surgery).The effective rate of improving TCM Syndrome Scale score was 91.2%.The above data were all tested statistical analysis by SPSS20.0 paired "t" test or rank sum test,P<0.05 is statistically significant for the difference.Conclusion:1.Buzheng Xugu Pill and external fixator for the treatment of elderly intertrochanteric fracture of femur has good anesthetic effect,short operation time,small trauma,reliable fixation and satisfactory clinical effect.In the treatment of external fixator for intertrochanteric fractures,the integrity of the external wall has a certain impact on the postoperative efficacy.2.In the treatment of intertrochanteric fracture with external fixator,the integrity of lateral wall has a certain influence on the curative effect after operation.3.There are complications of pinhole infection,stiffness of knee joint and loosening of fixation needle in the treatment of elderly femoral intertrochanteric fracture with external fixator,which should be prevented and dealt with in time according to the patient’s condition.4.Buzheng Xugu Pills can supplement liver and kidney,strengthen tendons and bones,promote fracture healing and improve Harris score,which needs further comparative study.
