| Objective:To explore the rule of traditional Chinese medicine in the inpatient medical records of clinical acute cerebral infarction,summarize the common medicine taste,extract the high-frequency combination of medicine and the experience of formula composition,and evaluate the effect of the empirical formula on the activity of PC12 cells after anoxia and glucose deficiency / reoxygenation.It provides a reference for further study on the treatment of acute cerebral infarction with traditional Chinese medicine,and for the inheritance and innovation of clinical application of traditional Chinese medicine.Methods:1.224 inpatient medical records with acute cerebral infarction diagnosed in the First Affiliated Hospital of Hunan University of traditional Chinese medicine from January 1,2018 to September 30,2019 were collected,175 prescriptions of traditional Chinese medicine were summarized,and the database was established by Microsoft Office Excel2007;the frequency distribution of drugs and cluster analysis between drugs were conducted by SPSS 25;IBM SPSS Modeler 18 to analyze the association rules of drugs;to analyze the four Qi and five flavor effects of drugs on the cloud platform of ancient and modern medical cases.2.Based on the CCK-8(cell counting kit-8)detection experiment,the optimal time of glucose reoxygenation of PC12 cells was selected.After that,the high,medium and low concentrations of serum containing Huoxue rongluoyan formula were intervened,and blank serum,model and normal group were compared.The OD value was measured by CCK-8 and the cell injury rate was obtained.Results:1.⑴frequency analysis: the top ten(frequency > 50%)of the173 traditional Chinese medicines involved are Caulis Spatholobi,Phoebe,Scrophularia,Ligusticum chuanxiong,Shengdi,red peony,Huangjing,Acorus calamus,hawthorn and earthworm.⑵Cluster analysis: the following main clusters were obtained: frankincense,myrrh,leech,turtle shell,tuckahen,Shichangpu;gastrodia,Pueraria,chuanniuxi;safflower,peach kernel,liquorice,white peony;Astragalus,dangshen,earthworm,tangerine peel,Fraxinus,Atractylodes rhizome,Poria cocos;Curcuma,trigonometry,Caulis Spatholobi,Photinia,Shengdi,Huangjing,chuanxiong,red peony,hawthorn,Puhuang,Xuanshen.⑶Association Rules: in the second-order association rules,the first three combinations of support degree are successively jixueteng,Shinan Teng,Shinan Teng,jixueteng,Xuanshen,Shinan Teng;in the third-order association rules,the first three combinations of support degree are successively Xuanshen+jixueteng,Shinan Teng,Shengdi+jixueteng,Shinan Teng,Chuanxiong+jixueteng,red peony;in the fourth-order association rules,the first three combinations of support degree are successively Shengdi+Shi Nanteng+jixueteng→Huangjing,Huangjing+jixueteng+shinanteng→Shengdi,Chuanxiong+shinant eng+jixueteng→red peony.⑷ In the total frequency of 2569,the highest frequency of four Qi is Wen 704,Ping 655 and Wei Han 369;the highest frequency of five flavors is Gan 1114,Ku 1073 and Xin 755;the highest frequency of the meridian is liver 1103,spleen 817 and lung 764;the main representative of the efficacy is nourishing Yin and clearing heat,relaxing muscles and activating collaterals,promoting blood circulation and removing stasis.2.⑴the injury rate of PC12 cells increased from 6h to 24 h after glucose deficiency and anoxia for 3h,and the maximum injury rate was 54.4% at24 h,and decreased from 24 h to 36 h.⑵PC12 cells were deprived of glucose and hypoxia for 3h/reoxygenated for 24 h.The cell injury rate of the two groups was lower than that of the control group.Conclusion:The main treatment of acute cerebral infarction patients is nourishing yin and clearing heat,relaxing muscles and activating collaterals,promoting blood circulation and removing blood stasis.The compatibility of rattan drugs such as Caulis Spatholobi and Phoebe is also its characteristics.The drugs are warm,gentle,cold and mainly into the liver.The combination of drugs for promoting blood circulation and removing blood stasis with drugs for relaxing muscles and activating collaterals,andthe combination of drugs for clearing heat and cooling blood with drugs for nourishing yin and activating blood all embody the idea of combining Qi and blood with both yin and Yang,and applying both attack and supplement.In the compatibility,we should pay attention to the drug taste,use the classic compatibility,and carry out innovative supplement,and obtain certain clinical treatment effect,which has a more in-depth mining value.In the preliminary study of cell experiment,we can know that CCK-8 has a protective effect on PC12 cells,and its effect increases with the increase of concentration,but its mechanism needs further analysis. |
