PurposeThis study is divided into clinical syndrome analysis and animal experiment.Clinical syndrome analysis aims to explore the main classification of TCM syndrome types of FD by collecting and sorting out the four diagnostic information of FD patients and conducting statistical calculation and processing.Animal experiments were conducted to compare the gastric emptying rate and the changes of serum MTL,Ghrelin and NPY in FD model rats by regulating liver and stomach,to explore the correlation between brain-intestinal peptide and FD,and to explore the possible mechanism of regulating liver and stomach in the treatment of FD.MethodsClinical syndrome study analysis: through collecting cases,150 cases of patients with FD Roman Ⅳ diagnosis standard, collection and its clinical information, statistical survey data, using statistical count bluff and adopt the method of factor analysis and rotating waiting the results of the survey analysis and processing.Experimental study: 60 SD rats were randomly divided into 4 groups(15 rats in each group)and named as normal group, model group, traditional Chinese medicine group and western medicine group. The normal group received routine feeding, and the other three groups received chronic stress stimulation for 21 days. After modeling, the normal group and the model group were given distilled water for control gavage daily, the traditional Chinese medicine group was given Tiao Gan He Wei Decoction gavage, and the western medicine group was given moxapride gavage for a total of 14 days. After the course of treatment, the general condition and body weight of the rats were observed, and the changes of gastric emptying rate and serum MTL,Ghrelin and NPY of the rats in each group were compared.Result1.Clinical study: Through statistical processing,a total of 10 main factors were extracted, and 7 types of syndromes were obtained through comprehensive analysis: stomach-qi obstruction syndrome,liver-stomach diastasis syndrome,liver-gallbladder and stomach heat retention syndrome,spleen-stomach damp-heat syndrome,spleen-stomach deficiency and qi stagnation syndrome,cold-heat disorder syndrome and spleen-stomach deficiency and cold syndrome(weak). The location of the disease mainly involves the stomach, spleen and liver, and the disease includes deficiency, deficiency and excess, involving pathological factors such as qi deficiency,Yang deficiency, cold, heat,qi stagnation and dampness and heat.2.experimental research:(1)After the modeling, the FD model rats showed dull and dry fur color, decreased aggression, slow behavior, loose stool, and rotten bedding. The weight gain of the three groups was significantly less than that of the normal group, indicating that the modeling was successful.(2)Effect of Tiao Gan He Wei Decoction on weight of rats: before modeling, the weight of rats in the four groups was relatively similar(P=0.251).After modeling, compared with the untreated normal group, the three groups of rats treated with the model were all lower in weight, with a significant difference(P<0.01).After treatment, the weight of rats in the traditional Chinese medicine group and the western medicine group was slightly higher than that in the model group, but the difference was not significant(P>0.05).(3)Effects of Tiao Gan He Wei Decoction on gastric emptying rate in rats: compared with the normal group, gastric emptying in the model group was significantly worse, with a significant difference(P<0.01),and gastric emptying in the western medicine group was also worse, with a difference(P<0.05).Compared with the model group, gastric emptying was significantly better in the traditional Chinese medicine group(P<0.01),but not in the western medicine group(P>0.05).Compared with the traditional Chinese medicine group, the gastric emptying in the western medicine group was significantly lower, with a significant difference(P<0.01).(4)Effects of Tiao Gan He Wei Decoction on serum MTL and Ghrelin in rats: compared with the normal group,MTL and Ghrelin levels in the model group were significantly reduced, with significant differences(P<0.01).Compared with the model group, the levels of MTL and Ghrelin in serum of rats in the traditional Chinese medicine group and the western medicine group were significantly higher(P<0.01).There were significant differences in MTL content and Ghrelin content between the two groups with drug intervention(P<0.01).(5)Effects of Tiao Gan He Wei Decoction on serum NPY in rats: compared with the normal group, the content of NPY in the model group was higher, and there was a significant difference between groups(P<0.01).Compared with the model group, the serum NPY content of the two groups of rats with drug intervention was significantly lower, with a significant difference(P<0.01).There was a significant difference in NPY content between the two groups with drug intervention(P<0.01).Conclusion1.The TCM syndromes of functional dyspepsia mainly include the following seven types: stomach-qi obstruction syndrome,liver-stomach diastasis syndrome,liver-gallbladder and stomach heat retention syndrome,spleen-stomach damp-heat syndrome,spleen-stomach deficiency and qi stagnation syndrome,cold-heat disorder syndrome and spleen-stomach deficiency and cold syndrome(weak). The location of the disease mainly involves the stomach, spleen and liver, and the disease includes deficiency, deficiency and excess, involving pathological factors such as qi deficiency,Yang deficiency, cold, heat,qi stagnation and dampness and heat.2.FD rats showed obvious slow gastric emptying, which may be related to the reduced secretion of MTL and Ghrelin,as well as the decreased role of MTL and Ghrelin in promoting gastrointestinal contraction and accelerating gastric emptying.Tiaogan and weifang may play a role in the treatment of FD by regulating MTL,Ghrelin and other brain-intestinal peptides.3.Liver and stomach regulation may reduce the content of NPY by slowing down stress-related reactions, thus improving FD-related symptoms.
