| Objective:Optic nerve atrophy is the final outcome of the development of a variety of optic nerve diseases.The main clinical manifestations are decreased vision or loss of vision,visual field defects,the color of optic disc become pale and can be found in the fundus examination.It is a serious blinding eye disease that threatens human health.The treatment is still a difficult problem in the world.Western medicine mainly deals with the cause of the disease,but there is no effective treatment afier the cause is removed.Acupuncture,as one of the characteristic treatment methods of traditional medicine,has achieved good results in clinical treatment of optic nerve atrophy,the improvement of visual electrophysiology in patients with optic nerve atrophy is particularly obvious,but there are few studies on the mechanism of acupuncture treatment of this disease,especially how acupuncture interferes with patients’ electrophysiology to improve visual function."Wei’s triple nine needling therapy.is an important prescription of Wei’s traditional Chinese medicine Ophthalmology in treatment of optic nerve disease.Previous studies have proven that it has a significant effect on the treatment of optic nerve atrophy.Therefore,this study explores the mechanism of the effectiveness of " Wei’s triple nine needling therapy" treatment of optic nerve atrophy from the perspective of visual electrophysiology.Providing evidence for the effectiveness of this therapy,Carrying forward and developing Wei’s ophthalmology academic idea,improving the clinical effect of optic nerve atrophy and benefiting the patients.Method:This trial used a prospective self-controlled trial method.According to the inclusion and exclusion criteria,35 patients with different causes of optic nerve atrophy were treated at Oriental Hospital from June 2019 to December 2019,those patients were treated with "Wei’s triple nine needling therapy",and Pattern Visual Evoked Potential examinations were performed before the acupuncture,immediately after the acupuncture de qi,needle retaining time of 30 minutes,and 10 minutes after the needle was pulled,and the P100 peak latent time and N75-P100 amplitude data,using single factor repeat measurement analysis of variance,to analyze the changes of Pattern Visual Evoked Potential at different time points.Results:①P100 peak latent time:At 1° and 15’spatial frequency,there were statistically significant difference in the P100 peak latent time between before the acupuncture,immediately after the acupuncture de qi,needle retaining time of 30 minutes and 10 minutes after the needle was pulled;The P100 peak latent time of 10 minutes after the needle was pulled at 1° spatial frequency and needle retaining time of 30 minutes at 15 ’ spatial frequency were significantly shortened compared with the time before acupuncture;The P100 peak latent time at immediately after the acupuncture de qi,needle retaining time of 30 minutes at 1° spatial frequency and immediately after the acupuncture de qi,10 minutes after the needle was pulled were trending shorter compared with the time before acupuncture,but the differences were no statistically significant;There were no statistically significant differences in P100 peak latent time between the two different time points after the acupuncture de qi.②N75-P100 amplitude:At 1° and 15’spatial frequency,there were no statistically significant difference in the N75-P100 amplitude between before the acupuncture,immediately after the acupuncture de qi,needle retaining time of 30 minutes and 10 minutes after the needle was pulled;The N75-P100 amplitude at each time point after the acupuncture de qi were trending higher compared with the time before acupuncture,but the differences were no statistically significant;There were no statistically significant differences in N75-P100 amplitude between the two different time points after the acupuncture de qi.Conclusion:Applying "Wei’s triple nine needling therapy" remedy the patient with optic nerve atrophy.Patients’ P100 peak latent time after acupuncture were improved compared with before the acupuncture,the effect of acupucture on the change of PVEP is facilitation,this change is considered to be related to the following factors:①Acupuncture immediately increases the excitability of the cerebral cortex and enhances the bioelectric activity in the visual centers.②Acupuncture affects the visual central neurotransmitter content,thus affecting the change of VEP;③It is also speculated that acupuncture improves the blood circulation of the optic nerve and temporarily improves the excitability of ganglion cells that are not completely damaged,the superposition of each acupuncture action can promote the repair of optic nerve and improve the conduction function of optic nerve;④And "de qi" may be the basis theory of TCM of acupuncture to produce immediate effects."Wei’s triple nine needling therapy" can improve optic nerve conduction function and protect incompletely damaged ganglion cells,providing a evidence for the effectiveness of this acupuncture therapy in the treatment of optic nerve atrophy. |
