Objective: To observe the effects of acupuncture on visual acuity,visual field and retinal nerve fiber layer thickness in patients with optic atrophy,so as to evaluate the clinical efficacy of acupuncture in patients with optic nerve atrophy.methods: From November 2019 to December 2021,30 patients with optic atrophy who met the standard in the Affiliated Hospital of Shandong University of traditional Chinese medicine were treated with acupuncture for three courses,and the self-control research method before and after treatment was adopted.The changes of best corrected visual acuity(BCVA),visual field MD,visual field VFI and the average thickness of retinal nerve fiber layer(RNFL)were compared before treatment and after each course of treatment.results:1.Thirty patients(47 eyes)with optic atrophy were included in this study.The male to female ratio was 7:3,The average age was(48.13±15.55)years,The most common cases with a course of 1-3 years are 12,and the cause is the most common due to glaucoma,with 10 cases.2.After three courses of treatment,the effective rate of visual acuity improvement was 63.83%;3.After treatment,the visual field MD of patients showed a gradual upward trend.There were significant differences in the second and third course of MD compared with the MD before treatment(P <0.05),there were significant differences in the second and third course of MD compared with the first course of MD(P <0.05),and there was no significant difference between the other two groups(P >0.05).It shows that there is no significant improvement in MD after one course of treatment,and there is significant improvement in MD after two courses of treatment.4.After treatment,the visual field VFI of patients showed a gradual upward trend.There was significant difference in VFI between the second and third courses of treatment(P <0.05),and there was significant difference in VFI between the third course of treatment and the first course of treatment(P <0.05),and there was no significant difference between the other two groups(P >0.05).It shows that there is no significant improvement in VFI after one course of treatment,and VFI can be significantly improved after two courses of treatment.5.After treatment,RNFLT showed a downward trend.There was significant difference in RNFLT between the first course of treatment and the third course of treatment(P <0.05),there was significant difference in RNFLT between the first course of treatment and the second course of treatment(P <0.05),and there was no significant difference in the other two courses(P >0.05).6.The effect of visual acuity improvement was not correlated with gender,course of disease and etiology,but correlated with age.There was significant difference in the constituent ratio of curative effect between patients aged 18-44 and patients aged ≥60(P <0.05).Conclusion: Although the atrophy and thinning of RNFL are still in progress during treatment,acupuncture can effectively improve the visual acuity and visual field(MD and VFI)of patients with optic atrophy.And the curative effect is closely related to the length of treatment time,the longer the course of treatment,the better the curative effect.Therefore,long-term adherence to treatment has a more positive significance for the improvement of patients’ visual acuity and visual field.It can be seen that even though the Retinal nerve fiber layer of patients with optic atrophy is still shrinking and thinning,acupuncture treatment can improve the visual function of patients with optic atrophy,improve visual acuity,visual field and quality of life to a certain extent.In addition,through the correlation analysis between visual acuity improvement effect and age,it is found that relatively young patients(≤44 years old)are more sensitive to acupuncture treatment and have better curative effect.
