| Objective:To explore the correlation and clinical significance of the serum HBV DNA load of patients with hepatitis B cirrhosis and the severity of the disease.Methods:Using retrospective analysis,clinical date of 504 patients with hepatitis B cirrhosis who were admitted to the Department of Gastroenterology of the Affiliated Hospital of Yanbian University from January 2010 to October 2019 were collected.General information,liver function,serum statistical analysis of HBV DNA load,etc,was divided into three groups of A,B,and C according to the Child-Pugh grading standard;at the same time,it was divided into two groups according to HBeAg positive and negative,and the correlation between serum HBV DNA load difference and liver function indexes were analyzed.Results:1.In 504 patients with hepatitis B cirrhosis,the average age,TBIL,ALB and serum HBV DNA load were different among different groups,and were statisticaliy significant(P<0.05).2.Serum HBV DNA load was positively correlated with ALT(rs=0.480,P<0.001),positively correlated with TBIL(rs=0.146,P<0.001),and negatively correlated with ALB(rs=-0.177,P<0.001).3.Child-Pugh improved grades A,B,and C have differences serum HBV DNA load ratios.In the Child-Pugh grades A,the low replication group is the main;while in the Child-Pugh grades B and C,the high replication group is the main;Compared with the ratio of HBV DNA>105IU/ml and HBV DNA<103IU/ml and HBV DNA 103-105IU/ml,Child-Pugh grade C group was higher than Child-Pugh grade A group,and the difference was statistically significant(P<0.05).4.Compared with HBeAg-positive group and HBeAg-negative group,serum HBV DNA load,ALT,and TBIL levels in HBeAg-positive group were higher than those in HBeAg-negative group,and ALB levels in HBeAg-positive group were sinnificantly lower than those in HBeAg-negative group,and the difference was statistically significant(P<0.05).5.Compared with the proportion of HBV DNA>105IU/ml in HBeAg positive group and the proportion of HBV DNA<103IU/ml and HBV DNA 103-105IU/ml in HBeAg positive group,the difference between HBeAg positivegroup and HBeAg negative group was statistically significant(P<0.01).Conclusion:1.There was a correlation between serum HBV DNA load and ALT,TBIL,and ALB.2.The serum HBV DNA load in patients with hepatitis B cirrhosis is related to the severity of the disease and the degree of liver dysfunction;the higher the serum HBV DNA replication level,the worse the liver function and the more serious the disease.3.Hepatitis B cirrhosis patients in the HBeAg-positive group had higher HBV DNA loads than those in the HBeAg-negative group;patients with hepatitis B cirrhosis in the HBeAg-positive group had more severe liver dysfunction than patients in the HBeAg-negative group. |
