ObjectiveAnalysis of acute and chronic lumbocrural Pain,Low Back Pain,LBP)in patients with lumbar MRI(Magnetic resonance image,MRI)of the digital data,evaluation of lumbar vertebral side muscle cross-sectional area(Cross-sectional area,CSA)and the change of the composition,researched on the relation between LBP and vertebral side muscle and its clinical significance.MethodsCases of this study is retrospective study data,collected between January 2018 and June 2018,all in guangdong hospital of ersha island orthopaedic hospital diagnosis of LBP,a total of 133 patients with unilateral,collect all of the patients with sex,age,height,weight,BMI,the use of hospital image archiving and communication system(PACS),measure and record all included in the sample of lumbar MRI image data,according to the duration of the pain can be divided into acute LBP and chronic LBP group,measurement of acute LBP and chronic LBP group on both sides of the crack more muscle(Musculus Fissure,MF),Psoas Major(Psoas Major,PM),vertical spinal muscular(Erector Spinae,ES)at CSAs,percentage of Fatty infiltration calculation bilateral muscle(Fatty infiltration,FI)and Lumbar sag values(Lumbar Indentation Value,LIV).Descriptive statistics were used to test the normal distribution of the two groups of samples,and the independent sample t test was used to compare the measurement data conforming to the normal distribution between the two groups.Mann-whitney-Wilcoxon test was used to compare the measurement data between the two groups that did not conform to the normal distribution to analyze whether there were differences in the paravertebral muscle CSA,FI and LIV in each group.ResultsAccording to the inclusion and exclusion criteria,a total of 133 patients were selected,including 58 patients in the acute group(32 males and 26 females)and 75 patients in the chronic group(52 males and 23 females).There was no significant difference in age,gender,weight,height and BMI between the acute group and the chronic group.Compared with the acute pain side,CSA of MF,PM and ES was smaller in the chronic pain side,with statistically significant difference(P<0.05).Compared with the acute non-pain side,MF was smaller in the chronic non-pain side,and the difference was statistically significant(P<0.05).No significant difference was found in the other groups.Compared with the acute pain side,FI of MF,PM and ES was larger in the chronic pain side,and the difference was statistically significant(P<0.05).Compared with the chronic non-pain side,ES was larger in the chronic pain side,and the difference was statistically significant(P<0.05).Compared with the acute non-pain side,MF was larger in the chronic non-pain side,and the difference was statistically significant.No significant difference was found in the other groups.Compared with the acute group,the chronic group had a smaller LIV,with a statistically significant difference(P<0.05).ConclusionLong-term persistent lumbago and leg pain can lead to muscle atrophy and lipogenesis of the lumbar vertebra,especially on the pain side.It was also found that paravertebral muscle atrophy and fat infiltration occurred simultaneously.Whether targeted lumbar and back muscle training at the early stage of LBP can help improve clinical symptoms remains to be further studied.
