| The health of the people is an important sign of the prosperity of the nation and the prosperity of the country.At present,the development of medical and health services in China is faced with more and more serious challenges such as the aging population and the high incidence of chronic diseases.On one hand,with the advancement of modernization and the rising incomes of the masses,people attach more and more importance to health,which leads the result that the past medical and health service model centering on hospitals and diseases can no longer meet people’s needs for long-term and continuous health care.On the other hand,residents are accustomed to go to the superior hospitals to see a doctor,which makes secondary and tertiary hospitals be overwhelmed,but the primary medical and health institutions are empty,as a result,the improvement of the overall medical environment,the balanced allocation of medical resources and the reasonable control of medical expenses are restricted severely.International experience and domestic practice prove that,it has become the general trend of the development of medical and health services and medical and health reform to establish a system of family doctors and promote contracted services.This paper takes C city of Sichuan province as an example.It selects family doctors contracted service as the research topic,takes stakeholder theory which involves the government and its departments,grass-roots medical and health institutions,family doctors and community residents as the research dimension,chooses institutional mechanism,policy guarantee,residents’ health awareness,family doctors service capacity and other projects as the primary and secondary research indicators,and guided by Maslow’s hierarchy of needs theory,fairness theory,aiming at analyzing the status,problems and causes of family doctors contracted service and putting forward the corresponding countermeasures.This paper is divided into five parts.The first part is the introduction,it explains the background of the topic and the significance of the research,reviews and evaluates the relevant studies at home and abroad,clears research ideas and methods,introduces the innovation and shortcomings.The second part is the concept definition and theoretical basis,it clearly defines the connotation and denotation of family doctors,contracted services and primary medical and health institutions involved in this paper,explains the main theoretical basis on which the paper was based,and builds the analytical framework.The third part is the status of contracted services in C city,Sichuan province,it mainly introduces the basic situation of C city,the basic situation of grassroots medical and health institutions,and the main methods of promoting contracted services.The fourth part is the analysis of problems and causes in the contracted service of C city,it reflects on the problems such as the low signing proportion,unbalanced signing structure,serious passive signing,"sign without engagement","about but not good",poor health management and so on in the process,and analyzes the cause of the problems from the community residents,family doctors,investment security,service capacity,system design and other aspects thoroughly.The fifth part is countermeasures and suggestions,it puts forward countermeasures and suggestions to further promote family doctors contracted service.And finally,it summarizes the study and the countermeasures and suggestions to promote contracted service in the conclusions and recommendations. |
