Clinical Research On EGF And TNF-α Of Diabetic Foot Ulcer By Combining Jiedu Xiyao Instillation With Vacuum Sealing Drainage

Object ive:By observing the effect of Jiedu xiyao combined with vacuum sealing drainage on epidermal growth factor(EGF)and tumor necrosis factor-α(TNF-α)on diabetic foot ulcer wound,the mechanism of promoting wound healing was explored.Methods:60 patients with diabetic foot ulcers from Department of Peripheral Vascular Diseases of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine were randomly divided into Jiedu xiyao group(observation group)and normal saline group(control group).They were given Jiedu xiyao or normal saline instillation in a 7±2-day therapy of vacuum sealing drainage respectively.The contents of EGF and TNF-α in granulation tissue before and after the operation of vacuum sealing drainage were detected,and the wound surface data were recorded.Observe two groups of EGF,TNF-α,the wound color,area,the drainage of content changes,and then discusses the promoting mechanism of action.Results:(1)On the 7th day after vacuum sealing drainage,the wound healing velocity and wound healing rate of the observation group were significantly higher than that of the control group(P<0.05).(2)The amount of granulation tissue and carrion in the wound and total score of wound symptom between the observation group and the control group are statistically significant(P<0.05).(3)On the 7th day after operation,the scores of wound color,wound exudate and periwound swelling increased.The difference was statistically significant immediately after operation(P<0.05).(4)The contents of EGF and TNF-α in the two groups immediately after vacuum sealing drainage were different from those before operation(P<0.05).(5)The evaluation of wound symptom efficacy:the total effective rate of the observation group was 63.3%,the total effective rate of the control group was 56.6%,and the difference was not statistically significant(P>0.05).(6)During the treatment,some patients were unable to bear the pain and the treatment was discontinued.In some cases,new ulcers appeared after vacuum sealing drainage was completed,and no other adverse reactions were found.Conclusion:The therapeutic effect of Jiedu xiyao combined with vacuum sealing drainage is better than that of normal saline combined with vacuum sealing drainage.The mechanism of action is related to the increase of EGF and the inhibition of TNF-αexpression.Combining Jiedu xiyao instillation with vacuum sealing drainage is an effective and safe method for diabetic foot ulcer treatment.
