| Objective: to observe the clinical effect of VSD combined with external application of Shengji Yuhong ointment on DFU.Methods: From December 31,2020 to December 31,2021,60 patients who met the standard of wound repair and peripheral vascular department in Wuhan Hospital of Integrated traditional Chinese and Western Medicine were selected.Collect patients’ sex,age,time of disease course before hospitalization,wound area,depth,test index and so on.Randomly divided into three groups,given systematic and standardized basic treatment,control of basic diseases and inflammatory infection,complete debridement in the operating room,VSD.The control group was treated with VSD alone（later referred to as group A）with VSD and routine wound dressing change.The first experimental group was VSD+ Shengji Yuhong ointment（later referred to as group B）,which was treated with VSD combined with Shengji Yuhong ointment.The second experimental group was VSD+PRF group（C group）,which was treated with VSD combined with platelet-rich fibrin dressing.The total effective rate,cure rate,hospitalization days,ulcer area and exudation score before and after 4 weeks treatment were recorded and compared among the three groups.Results: there were no shedding cases in this study,and the treatment study of 60 patients was completed.（1）Efficiency comparison:After 4 weeks of treatment with their respective treatment plans,the total effective rate of group A was65.0%.The total effective rate of group B was 95.0%.The total effective rate of group C was 100%.After treatment,the wounds of the three groups were better than that of admission,and the effective rate of BC and BC was higher than that of group A（P<0.05）.（2）Comparison of cure rate:After 4 weeks of treatment,the cure rate of group A was 10.0%,the cure rate of group B was 35.0%,and the cure rate of group C was 45.0%.The cure rate of the two groups was better than that of group A,and there was significant difference among the three groups after 4 weeks of treatment（P<0.05）.（3）Comparison of hospitalization days:The hospitalization days of group A,B and C were 44.50（35.75～58.50）,37.00（25.25～44.00）and29.00（25.00～31.75）,respectively.There was significant difference in hospitalization days among the three groups（P<0.01）.Group C was shorter than group B and group A.（4）Comparison of wound area:The wound area of the three groups before and after treatment was significantly smaller than that before treatment（P<0.001）.In comparison between groups,the reduction of wound area in group B and C was better than that in group A（P<0.001）,and there was no significant difference in BC between the two groups（P>0.001）.（5）Exudation score comparison:After 4 weeks treatment,the exudation of wound secretion in the three groups was significantly improved as compared with that before treatment（P<0.001）.After treatment,group C was better than group B and group A（P<0.05）.（6）Adverse events:Blockage of drainage tube occurred in 1 patient in group B.Postoperative negative pressure leakage occurred in 2cases in group A,2 cases in group B and 3 cases in group C.There was no difference among the three groups（P>0.05）.Conclusion:The treatment schemes adopted by the three groups of patients are all effective for DFU.VSD combined with Shengji Yuhong ointment is superior to VSD alone in promoting wound healing and shortening hospitalization days in DFU.And its curative effect and course of treatment are basically the same as those of the PRF group. |
