| Objective: to understand the changes of health human resources in Jilin Province from2011 to 2019,evaluate the fairness of the allocation of health human resources in Jilin Province,and put forward reasonable suggestions on the allocation of health human resources in Jilin Province.to provide effective theoretical support for further optimizing the allocation of health human resources in Jilin Province in the future.Methods: based on the previous literature in related fields,the theories of health human resources applied in this study were reviewed,and the research on the allocation of health human resources at home and abroad was collated and summarized.the quantity and distribution of health human resources during the year of this study were descriptively analyzed,and the Lorentz curve,Gini coefficient and Theil index were drawn and calculated according to the research data.the fairness of health human resources allocation in Jilin Province was analyzed by comparing the corresponding standard boundary points.On this basis,this study proposes a new fairness analysis scheme,which combines the artificial intelligence algorithm Random Forest,to jointly judge the multi-dimensional data on the basis of the traditional fairness analysis.At the same time,in the acquisition of data sets,through a large number of collection and data processing,it is possible for the implementation of this method;from the results,this method not only complements the shortcomings of traditional fairness analysis,but also is more in line with China’s national conditions in terms of efficiency and practicality.Results: 1.Changes of Health Manpower allocation in Jilin Provincefrom 2011 to 2019,there was a significant increase in the total number of health workers except rural doctors and health workers in Jilin Province.By 2019,in the comparison between urban and rural areas,the number of health personnel in cities is generally higher than that in rural areas in terms of the number of health personnel and the number of health personnel per thousand population,but the difference is not significant.The results of the study marked by regions(8 prefecture-level cities and 1 autonomous prefecture)show that Changchun,the provincial capital,accounts for the largest proportion of health personnel,while Liaoyuan City and Baishan City account for the smallest proportion.Jilin Province has the highest performance in the number of health technicians per thousand population,while Baicheng City has the lowest performance.Compared with the other two provinces in Northeast China,the increase of all kinds of health workers in Jilin Province from 2011 to 2019 is obviously due to Heilongjiang Province and Liaoning Province.Among the seven provinces in the central region,Jilin Province does not have a distinct advantage and ranks relatively behind as a whole,but ranks second in the number of practising(assistant)doctors per thousand population with 2.9 person-times,indicating that Jilin Province is relatively reasonable in the allocation of the number of doctors in the province.2.Fairness of Health Manpower allocation in Jilin Province.(1)the results of this study combined with Lorentz curve and Gini coefficient analysis show that the overall fairness of health manpower in Jilin Province in the past nine years is better,and the fairness of licensed(assistant)doctors is better than that of registered nurses.Registered nurses and rural doctors showed a more obvious trend in the development of fairness,and the development of fairness of managers showed a downward trend from 2011 to 2019.(2)the Theil index of health manpower allocation in Jilin Province has generally shown a downward trend in the past nine years,in which the Theil index of registered nurses is the highest.by disassembling the Theil index to observe the differences between regions and within regions,we can see that the differences in the fairness of the allocation of doctors and nurses are mainly due to inter-regional differences,and rural doctors and administrators mainly come from intra-regional differences,but the interval differences of managers are increasing year by year.(3)before 2017,the result of the rationality model of overall health resource allocation in Jilin Province shows that the fairness of Jilin Province needs to be improved compared with other provinces and cities in China.However,since 2017,the rationality of the overall allocation of health resources in Jilin Province has gradually improved.In recent years,under the same level of other dimensions,health personnel,hospitals and grass-roots medical and health institutions have increased significantly.The overall rationality of medical distribution has also been highly improved.Conclusion: the overall number of health workers in Jilin Province developed well from 2011 to 2019,but there is still a large gap compared with other provinces in Northeast China and other provinces in the central region of the economic region.and a large number of health manpower are mainly concentrated in cities with better economic development,such as Changchun,the provincial capital,so the quantity and distribution of health manpower in Jilin Province still need to be further improved.The fairness of health manpower in Jilin Province is better as a whole,and there is still some room for development compared with other provinces and cities in China.the increase in the fairness of the allocation of nurses among health technical personnel is obvious,so it is necessary to pay more attention to the fairness of the allocation of management personnel. |
