| Objective:To observe the analgesic effect of dexmedetomidine combined with ropivacaine serratus anterior block in the perioperative period of open surgery for liver cancer.Method:Fifty patients undergoing laparotomy for liver cancer admitted to the First Hospital of Jilin University from July 2020 to January 2021were selected as the research objects,and the random number table method was adopted to divide them into a control group and a test group,with 25 cases in each group.Serratus anterior plane block was performed in both groups.The control group was injected with 0.25%ropivacaine solution 40ml,and the test group was injected with 0.25%ropivacaine and 1ug/kg dexmedetomidine mixed solution totaling 40ml.Comparison of changes in blood pressure and heart rate at 5 minutes before and after skin incision and T1(2h after extubation),T2(6h after extubation),T3(12h after extubation),T4(24h after extubation),T5(36h after extubation))And T6(48h after extubation)visual analogue pain score(VAS score),and intravenous patient-controlled analgesia during 0-2h,2-6h,6-12h,12-24h,24-36h,and 36-48h after extubation The use of the pump and adverse reactions.SPSS 26.0 statistical software was used to analyze the results,mainly using?~2test,non-parametric rank sum test,independent sample t test,and paired sample t test to compare the general data of the two groups.P>0.05 is considered to be insignificant,P<0.05 is considered to be statistically significant,and P<0.01 is considered to be very statistically significant.Results:1.Baseline characteristicsThere was no significant difference between the experimental group and the control group in terms of age,gender,height,weight,BMI,systolic blood pressure,diastolic blood pressure,heart rate and operation time(P>0.05).2.Blood pressure and heart rate at 5 minutes before and after skin incisionThere was no significant difference in blood pressure and heart rate changes between the test group and the control group at 5 minutes before and after the skin incision(P>0.05).3.VAS score after extubating(1)The VAS scores[median(25%,75%)]of the experimental group and the control group at T1 were 1(1,2)and 2(1,2)(P=0.291);(2)The VAS scores[median(25%,75%)]of the experimental group and the control group at T2 were 2(1,2)and 2(2,2)respectively(P=0.457);(3)During T3～T6,the visual analogue pain score(VAS)of the test group was lower than that of the control group,and the difference between the two groups was statistically significant(P<0.05).4.Number of uses of intravenous self-control analgesia pump after extubating(1)The number of times[median(25%,75%)]used intravenous patient-controlled analgesia pump during 0-2h after extubating in the experimental group and the control group were 0(0,1)and 1(0,1),respectively(P=0.186);(2)The number of times[median(25%,75%)]used intravenous patient-controlled analgesia pump during 2-6h after extubating in the experimental group and the control group were 1(0,1)and 1(1,1),respectively(P=0.121);(3)The number of times of using self-controlled intravenous analgesia pump during 6-12h,12-24h,24-36h,36-48h after operation in the control group was lower than that in the control group,and the difference between the two groups was statistically significant(P<0.05).5.Adverse reactions:The number of nausea and dizziness in the experimental group was significantly less than that in the control group,and the adverse reactions were mild.Conclusion:Ultrasound-guided dexmedetomidine combined with ropivacaine serratus anterior block has a good analgesic effect in open surgery for liver cancer,which can effectively relieve the postoperative pain of patients,reduce the additional analgesic drugs,and at the same time reduce the incidence of adverse reactions,shortens the hospital stay and the prognosis is good. |
