| Object:Through the clinical observation of Baduanjin combined with metformin in the treatment of polycystic ovary syndrome(PCOS)of spleen deficiency and phlegm dampness type,to explore its curative effect analysis and possible mechanism of action,so as to open up a new idea for TCM exercise intervention of polycystic ovary syndrome,in order to serve the clinical treatment.Methods:From May 2020 to December 2020,41 patients with polycystic ovary syndrome(PCOS)of spleen deficiency and phlegm dampness type were enrolled in the Department of Endocrinology,Nanjing Drum Tower Hospital.41 patients were randomly divided into two groups:the treatment group(Baduanjin combined with metformin)and the control group(walking combined with metformin)for 12 weeks.After treatment,BMI,TCM syndrome score,sex hormone related indexes(FSH,LH,LH/FSH,t,SHBG,Fai,AMH),islet function indexes(FPG,2hPG,fins,2hins,HOMA-IR),blood lipid levels(TC,TG,LDL-C,HDL-C)and ovarian volume under ovarian ultrasound were observed.Results:Before treatment:there was no significant difference in BMI,TCM syndrome score,sex hormone level(FSH,LH,LH/FSH,t,SHBG,Fai,AMH),islet function index(FPG,2hPG,fins,2hins,HOMA-IR)and blood lipid level(TC,TG,LDL-C,HDL-C)between the two groups.After treatment:The TCM syndromes in the treatment group and the control group were improved,and the improvement in the treatment group was more obvious than that in the control group(t=-4.049,P=0.000).The specific difference showed that the average value of the treatment group(5.60)was significantly lower than that of the control group(9.05).The hormone levels of the two groups were improved in varying degrees.In terms of improving SHBG and FAI,there were significant differences between the two groups before and after treatment,but there was no significant difference between the two groups(P>0.05),indicating that the two groups had consistency in these indicators,and the curative effect was similar.On the level of AMH,there was no significant difference between the two groups after treatment,but the specific comparison showed that there was a significant difference in the treatment group before and after treatment(P<0.05),while there was no significant difference in the control group(P>0.05).In the T level,the two groups before and after treatment did not show significant difference,but the specific comparison shows that the average value of the two groups decreased.There was no significant difference in FSH,LH and LH/FSH between the two groups before and after treatment(P>0.05).In terms of islet function level,at the level of 2hins,there was a significant level of 0.05 between the two groups after treatment(t=-2.047,P=0.049).The specific comparison difference showed that the average value of the experimental group(68.27)was significantly lower than that of the control group(88.83).FINS and HOMA-IR of the two groups were significantly lower than those of the treatment group,but there was no significant difference between the two groups(P>0.05).In terms of lipid metabolism,TG,TC and LDL-C in the treatment group and the control group were decreased in varying degrees before and after treatment,the difference was statistically significant(P<0.05),and there was no significant difference between the two groups(P>0.05);in addition,HDL-C in the two groups was not significant before and after treatment,the difference was not statistically significant(P>0.05).ConcLusion:Baduanjin is more effective in treating PCOS patients with spleen deficiency and phlegm dampness type than walking at medium walking speed.It is effective in relieving TCM clinical symptoms,reducing AMH and improving islet function.Due to walking,SHBG,FAI and lipid metabolism levels are significantly improved,but the curative effect is similar to walking.In the process of practicing Baduanjin,there was no obvious adverse reaction.Therefore,this study provides a new treatment idea for clinical TCM non drug intervention in the treatment of polycystic ovary syndrome. |
