Clinical Study On Rehabilitation Effect Of Lower Limb Closed Kinetic Chain Exercise On Patients With Knee Osteoarthritis

Objective:To observe the effects of closed kinetic chain exercise of lower limbs on the function and quality of life of patients with knee osteoarthritis through clinical randomized controlled trials,and to provide clinical research evidence for the treatment of knee osteoarthritis.Methods: Sixty-four patients with knee osteoarthritis admitted to the rehabilitation department and inpatient department of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from October 2019 to December 2020 were selected as the research subjects,and randomly divided into a combined group and a control group,with 32 patients in each group.The control group was given health education,physical agent therapy and routine exercise therapy,while the combined group was given lower limb closed chain exercise training on the basis of the control group.Except for short-wave therapy 3 times/week and interference electric therapy 5 times/week,other treatments were given 10 times/week for 4 weeks.Before and after treatment,WOMAC scale,peak torque and active and passive joint position perception errors were used to evaluate the two groups,and the evaluation results were compared and analyzed.Results: Before treatment,there were no significant differences in WOMAC scale score,peak torque and active and passive joint position perception error between 2 groups(P<0.05).After treatment:(1)WOMAC scale in 2 groups was significantly lower than before treatment(P<0.01),pain,difficulty of daily living score and total score in combined group were lower than those in control group(P<0.05),stiffness score in combined group was significantly lower than that in control group(P<0.01).(2)Compared with before treatment,the peak torque of 60°/s and 180°/s extensor and flexor in 2 groups was significantly increased(P<0.01),and the improvement of 60°/s and 180°/s flexor and extensor in combined group was better than that in control group(P<0.05).(3)The error of active and passive position in both groups was significantly lower than that before treatment(P<0.01),and the error of active and passive position in combined group was lower than that in control group(P<0.05).Conclusion: Lower extremity closed kinetic chain exercise training combined with physical agent therapy and routine exercise therapy can effectively relieve knee stiffness and pain in patients with knee osteoarthritis,improve muscle strength around the knee joint,improve proprioception,and reduce the difficulty of daily living activities.The results were better than physical agent therapy combined with conventional exercise therapy.
