| Objectives:Divided 53 patients with septic cardiomyopathy into three groups.After administering dexmedetomidine(dex),Shenfu injection,and the combination of the two,the inflammatory indexes,myocardial injury indexes,and indexes of each group were observed.Changes of cardiac function indexes and ICU stay before and after treatment,to explore the short-term clinical efficacy of dexmedetomidine combined with Shenfu injection in the treatment of septic cardiomyopathy,and provide clinical evidence for further treatment of patients with septic cardiomyopathy.Methods:Using prospective research methods,53 patients with septic cardiomyopathy who met the standards in the ICU of the Three Affiliated Hospital of North Sichuan Medical College from November 2019 to November 2020 were selected as the research objects,and they were divided into dexmedetomidine by a random number table.Group(n=18),Shenfu injection group(n=18),Dex combined with Shenfu injection group(n=17).Collect the general clinical data of patients,observe the three groups of patients before treatment and 3,7 days after treatment APACHE Ⅱ score,stay in ICU,myocardial injury markers(BNP,cTnI,CK-MB),inflammatory indicators(PCT,IL-6),TNF-α),changes in cardiac function indicators(CI,CO,SV,SVV).All data are statistically analyzed using SPSS21.0.Compare the differences in general data of each group.The measurement data conforming to the normal distribution are represented by the mean±standard deviation(x±s),and repeated measurement analysis of variance is performed;the measurement data not conforming to the normal distribution are all taken as the median(quartile)(M(QL,QU))Indicates that the generalized estimation equation is used for analysis.Enumeration data is expressed by(%),and comparison between groups is by χ2 test;the test level is P<0.05,which indicates statistical significance.Results:(1)The APACHE Ⅱ scores of the three groups of patients were significantly reduced 3 and 7 days after treatment,and the right Medetomidine combined with Shenfu injection group was significantly lower than that of Shenfu injection group and Dex group,and the difference was statistically significant(P<0.05);patients in the combined group stayed in ICU for significantly shorter time than the Shenfu group and Dexmetol The dexmedetomidine group was significantly shorter in the middine group and the participating group,and the difference was statistically significant(P<0.05).(2)The IL-6 and TNF-a indicators of the three groups of patients were significantly improved at 3 and 7 days after treatment,and the improvement of Dex combined with Shenfu injection group was significantly better than that of Shenfu injection group and dexmedetomidine In the Shenfu injection group,the Shenfu injection group was significantly better than the dexmedetomidine group,and the differences were statistically significant(P<0.05);(3)The BNP and cTnI of the three groups of patients were significantly improved 3 and 7 days after treatment,and the improvement of Dex combined with Shenfu injection group was significantly better than that of Shenfu injection group and dexmedetomidine group.Shenfu injection The liquid group was significantly better than the dexmedetomidine group,and the differences were statistically significant(P<0.05).(4)The CI,SV,CO,and SVV indicators of the three groups of patients were significantly improved 3 and 7 days after treatment,and the improvement of Dex combined with Shenfu injection group was significantly better than that of Shenfu injection group and dexmedetomidine Group,Shenfu injection group was significantly better than dexmedetomidine group,the difference was statistically significant(P<0.05).Conclusion:1.Dexmedetomidine combined with Shenfu injection reduces the level of inflammation in the body,improves septic myocardial injury,hemodynamic parameters,and myocardial function.Compared with the Shenfu group and Dex group,the effect is better;while Shenfu injection reduces The level of body inflammation and the improvement of myocardial damage are better than Dex;2.Dexmedetomidine combined with Shenfu injection reduces the severity of SCI more significantly;3.Dexmedetomidine combined with Shenfu injection significantly reduces the time spent in ICU compared with dexmedetomidine and Shenfu injection... |
