Objective: The preferred surgical method for chronic subdural hematoma(chronic subdural hematoma CSDH)is drilling and drainage,but the postoperative residual blood absorption is slow,the recovery period is long,and the recurrence rate is high.Atorvastatin plays a role in the treatment of CSDH.The specific dose of statins in CSDH patients has not yet been unified.This study analyzed the effect of different doses of atorvastatin combined with drilling and drainage in the treatment of chronic subdural hematoma to further determine the choice of postoperative atorvastatin dose,In order to better guide clinical medication.Methods: A selection of 72 patients who were admitted from August 2019 to December 2020 and met the "Neurosurgery Course" diagnostic criteria for CSDH were randomly divided into two groups: A(experimental group)and B(control group),and group A(experimental group)36 patients were given atorvastatin 40 mg from the first day after surgery and were taken every night before going to bed.B(control group)36patients were given atorvastatin 20 mg from the first day after surgery and were taken every night before going to bed.Head CT was reviewed regularly,and the efficacy,hematoma volume before and after treatment,Markwalder classification,recurrence,and adverse reactions during treatment were assessed after both groups were taken for 3months.Results: 72 patients were grouped and compared:(1)There was no statistical difference between the experimental group and the control group in admission gender,age,hematoma volume,preoperative hematoma side,and Markwalder grade(P<0.05).(2)The total effective rate of the experimental group was 91.6%(33/36),which was significantly higher than the control group’s 72.2%(26/36).The difference was statistically significant(p<0.05)(2)The experimental group was 3 The volume of hematoma at 1 month was 10.33±2.34;the volume of hematoma at 3 months after operation in the control group was 15.72±4.35.Compared with the control group,the amount of hematoma in the experimental group was significantly reduced 3 months after surgery(P<0.05).(3)The Markwalder classification of patients in the experimental group was significantly better than the control group(P<0.05).(4)There were 1 cases of recurrence in the experimental group and 6 case in the control group,which was statistically significant(P<0.05).During the experiment,the patient did not experience atorvastatin-related adverse reactions,including nausea and vomiting,diarrhea,abnormal liver function,abnormal blood routine,myalgia,skin itching,etc.Conclusion: Based on the drilling and drainage of chronic subdural hematoma,atorvastatin 40 mg can promote the absorption of residual blood and nerve function recovery in patients with chronic subdural hematoma compared with 20 mg,and reduce the recurrence rate after CSDH.
