| Objective:To explore the relationship between serum chemerin levels in patients with Type 2 diabetes mellitus(T2DM)and abdominal obesity and the development and progression of atherosclerosis.Methods:This study included 117 inpatients with T2DM from the Endocrinology Department of the First Affiliated Hospital of Bengbu Medical College from June 2020 to February 2021.All research subjects collected data,including general examinations:age,sex,disease course,blood pressure,height,weight,body mass index(BMI),etc.Clinical indicators:fasting blood glucose(FBS),fasting insulin(FINS),glycosylated hemoglobin(Hb A1c),total cholesterol(TC),triglycerides(TG),low-density lipoprotein-cholesterol(LDL-C),high Density lipoprotein-cholesterol(HDL-C),C-reactive protein(CRP)),chemerin,abdominal visceral fat area(VFA),abdominal subcutaneous fat area(SFA)and carotid intima-media thickness(CIMT).According to the CIMT grouping,CIMT<0.9 mm was considered as the normal CIMT group(n=55),and CIMT≥0.9 mm was considered as the CIMT thickening group(n=62).According to the diagnostic criteria of abdominal obesity,BMI≥25 and VFA≥100 cm2were abdominal obesity group(n=49),and the rest were non-abdominal obesity group(n=68).We use SPSS 22.0 statistical software for statistical analysis of the data.Results:Compared with the normal CIMT group,the TC,TG,FINS,HOMA-IR,DBP,SBP,VFA,and Chemerin was increased in the CIMT thickening group(all of it was p<0.05);compared with the non-abdominal obesity group,TC,TG,HOMA-IR,DBP,SBP,SFA、CIMT and Chemerin was increased in the abdominal obesity group(all of it was P<0.05).The results of correlation analysis showed that serum Chemerin in patients with Type 2 diabetes mellitus was shown a positive correlation with TC,TG,BMI,FINS,HOMA-IR,VFA,SFA and CIMT(r were 0.377,0.433,0.376,0.246,0.780,0.756,0.323 and 0.593 respectively,P<0.05 or P<0.01).The results of binary logistic regression analysis showed that VFA,Chemerin,HOMA-IR,SBP and TG were the influencing factors of CIMT.Chemerin and SFA are the influencing factors of VFA.ROC curve analysis showed that when Chemerin combined with VFA predicted CIMT thickening,the area under the curve was 0.914,the sensitivity was 93.3%,and the specificity was 84.6%.Conclusion:Chemerin can promote the thickening of CIMT in T2DM patients,which is a risk factor and is involved in the occurrence and development of AS in T2DM patients.Chemerin promotes the occurrence of abdominal obesity in T2DM patients.Chemerin combined with VFA can be used as a predictor of CIMT thickening in T2DM patients. |
