Objective: The study was to discuss the effect of vitamin D(VD)metabolism on creatine kinase(CK)in patients with type 2 diabetes mellitus(T2DM).Methods: After the inclusion and exclusion criteria,150 T2DM patients who were treated at Shiyan Dongfeng General Hospital of Sinopharm from October 2018 to January2020 were selected,According to serum 25(OH)D3 level,150 T2DM patients were divided into VD deficiency group(n=84),insufficiency group(n=44),sufficiency group(n=22),Collect basic information(sex,age,course of disease,height,weight)and Glycosylated hemoglobin(HbA1c),VD,CK,Triglyceride(TG),High-density lipoprotein cholesterol(HDL-C),Low density lipoprotein cholesterol(LDL-C)and other blood biochemical indicators,compare the general clinical data and biochemical indicator levels between the three groups.Analyze the correlation between VD and CK in patients with different genders,different ages and different body mass index(Body Mass Index,BMI).Analysis of the influencing factors of creatine kinase with multiple linear regression.Results:(1)Among 150 T2DM patients,there were 84 cases(56%)in the VD deficiency group,44(29.3%)in the VD deficiency group,22(14.6%)in the VD sufficient group,and 128(85.3%)in the vitamin D deficiency and deficiency group.(2)The P value of vitamin D among different gender,age and BMI groups were all greater than 0.05,there was no significant difference.(3)The levels of CK and VD in T2DM patients of different gender groups and different age groups were negatively correlated(P<0.05).T2DM patients in the overweight and obese group were also negatively correlated,there was no significant relationship between T2DM patients in the normal weight group.(4)The prevalence of diabetic nephropathy is different among different vitamin D groups(P<0.05).(5)There are statistical differences in CK and HbA1c between different vitamin D groups(P<0.05).The comparison between the two groups found that the levels of CK and HbA1 c in the vitamin D deficiency group were higher than those in the vitamin D deficiency group and the adequate group [CK(U/L): 120(84.25,169)vs 80.5(66.75,118.25),105(75,121.5),HbA1c(%): 9.2(7.82,10.97)vs 8.4(7.52,9.95),7.8(7.27,9.6),P<0.05)].(6)Multiple linear regression analysis showed that VD,BMI,and HbA1 c are the influencing factors of CK(β=-0.209,0.198,0.161,P<0.05 or P<0.01).And the linear regression equation can be drawn: Y=-9.357-0.568X1+4.316X2+5.188X3.At the same time,we can see that the VIF values of collinearity diagnosis are all less than 10,there is no multicollinearity in the regression model,and there is no linear correlation between the independent variables.According to the ANOVA table,it can be seen that the regression mean square = 24396.517,the residual mean square = 3244.175,the model F statistical value is 7.520,and the significance P<0.01.It indicates that the regression model considers that the linear equation fitted by the regression model has statistical significance through the F test with a confidence level of 0.05,there is a straight line relationship between the dependent variable Y and the independent variable X.Conclusion:(1)Vitamin D deficiency or insufficiency is very common in patients with T2DM.(2)The reduction of VD in overweight and obese T2DM patients may cause an increase in CK.There was no significant correlation between vitamin D and CK in T2DM patients with normal weight.(3)The lack of vitamin D in patients with T2DM may increase the levels of creatine kinase and HbA1 c.(4)The decrease of vitamin D level in T2DM patients may increase the prevalence of diabetic nephropathy.(5)The linear regression equation between creatine kinase and vitamin D,BMI,and HbA1 c is: Y=-9.357-0.568X1+4.316X2+5.188X3.It can be seen that there are significant differences between creatine kinase and vitamin D,BMI,and HbA1 c.Influence the relationship.
