| Objective: To explore the clinicopathological significance of glomerular C4 d deposition in IgA nephropathy.Methods: The study included 280 patients with primary IgA nephropathy diagnosed by renal biopsy from January 2015 to December2018.The patients were dividedinto the C4 d positive group and the C4 d negative group according to the C4 d score results of the glomerulus,and the differences in clinical and pathological indicators were compared between the two groups.Results: 1.C4 d positive group of the 24 h urinary protein,total cholesterol,low-density lipoprotein,high blood pressure ratio,the proportion of patients with 1 g higher in the 24 h urinary protein than C4 d negative group(P < 0.05),while serum albumin,e GFR levels below C4 d negative group(P < 0.05).The proportion of renal tubular atrophy/interstitial fibrosis(T1/2),the proportion of Lee’s pathological grade III to V and Ig M deposition in C4 d positive group were all higher than those in C4 d negative group.Conclusions: IgAN patients with C4 d deposition in the glomerulus have more severe clinicopathological manifestations,indirectly confirming that C4 d deposition in the glomerulus may be a risk factor affecting the prognosis of IgA nephropathy.Clinically,it is recommended that such patients should be followed up closely and treated with early intervention. |
