| Objective:To investigate the efficacy of 3D printing combined with an anatomical Distal Volar Radius(DVR)plate in the treatment of complex Distal radial fractures..Methods: A retrospective study was performed on 77 patients diagnosed with distal radius fractures in our hospital between July 2018 and February 2020.A total of 48 patients with complex distal radial fracture were screened out.According to the application of 3D printing technology,24 cases of 3D printing combined with DVR anatomical locking plate treatment were regarded as the experimental group,and 24 cases of imaging combined with DVR anatomical locking plate treatment were regarded as the control group.The amount of intraoperative blood loss,operative time,intraoperative fluoroscopy times,postoperative imaging,postoperative wrist function and complications in each group were analyzed and compared.Chi-square test was used to analyze the inter-group rate comparison,and P<0.05 for all test criteria was considered statistically significant.Results:The excellent and good rate of postoperative efficacy in the experimental group(87.5%)was significantly higher than that in the control group(70.8%),P=0.047,and the difference was statistically significant.The operative time,intraoperative blood loss and intraoperative fluoroscopy times in the experimental group [(62.00±3.75)min,(25.00±1.00)ml,(3.00±1.00)times] were better than those in the control group [(69.50±3.75)min,(27.00±3.00)ml,(5.00±1.00)times].P < 0.05,statistically significant;Image: Postoperative palmar inclination(12.50±1.00)° in the experimental group was higher than that in the control group(11.00±1.00)°,P=0.000,and the difference was statistically significant.However,there were no significant differences in ulnar deviation Angle and radial styloid height [(22.00±1.00)°,(12.00±1.00)mm]between the experimental group and the control group((22.00±1.75)°,(12.00±0.01)mm](P>0.05).The functional range of wrist in test group 3months after surgery(palmar flexion,dorsiflexion,radial deviation,ulnar deviation,pronation,supination)[(66.00±3.75)°,(66.00±5.75)°,(36.00±3.00)°,(20.50±1.00)°,(86.00±2.75)°,(80.50±1.00)°)] °] was higher than that of the control group(60.00±2.00)°,(58.00±4.25)°,(32.00±1.00)°,(17.50±1.00)°,(81.50±2.00)°,(76.00±2.75)°).The difference was statistically significant(P<0.05),but the ratio of grip strength on the affected side to the healthy side in 2 groups was(80.00±13.75)% compared with that of the control group(80.00±15.00)%,P=0.194,showing no statistical significance.In this study,Fisher’s exact probability method was used to compare and analyze 24 patients in the experimental group.Patients whose operation time was less than or equal to 6 days had better efficacy at 6 months follow-up than patients whose operation time was greater than 6 days,P=0.032,which was statistically significant.Fracture type had an impact on postoperative efficacy,P=0.044,which was statistically significant.Conclusion:1 3D printing combined with DVR anatomical locking plate therapy reduces the operative time,intraoperative bleeding and the number of intraoperative fluoroscopy,and also improves the excellent and good rate of postoperative efficacy.2 The timing of operation and fracture type influence the postoperative outcome of 3D printing combined with DVR anatomical locking plate in the treatment of patients with complex distal radial fractures.3 3D printing combined with DVR anatomical locking plate in the treatment of complex distal radial fractures is more precise and personalized. |
