Objective Observe the effects of different doses of dexmedetomidine on the implicit and explicit memory of elderly patients undergoing hip and knee replacement surgery under spinal anesthesiaMethod 1.A total of 54 elderly patients(≥65 years old)who met the inclusion criteria and exclusion criteria for hip and knee replacement surgery under spinal anesthesia in our hospital from January 2020 to January 2021 were collected.2.54 patients were randomly divided into three groups(D1 group,D2 group and D3 group),18 patients in each group.All patients underwent wake-listening word learning immediately after the anesthesia plane fixation and meeting the surgical requirements.Then,dexmedetomidine was pumped into the three groups at the rates of 0.2ug/(kg·h),0.4ug /(kg·h)and 0.6ug/(kg·h),Sedation word learning was performed 1 hour after pumping DEX,and dexmedetomidine pumping was stopped 30 minutes before the end of surgery.MAP、HR and Sp O2 were recordde before anesthesia,at plane fixation,pumping for 15,30 and 60 minutes,and at the end of surgery,Four hours after the operation,the results of implicit and explicit memory were obtained according to the memory processing separation procedure and stem-filling experiment.3.Statistical methods were used to analyze the difference between the scores of implicit and explicit memory and zero,as well as the differences of the implicit memory scores of three groupsResult In group D1,patient’s implicit memory and explicit memory scores was statistically and significantly different from zero(P < 0.05).In group D2,patient’s implicit memory scores was statistically and significantly different from zero(P <0.05)but their explicit memory scores was not statistically and significantly different from zero(P > 0.05).And the scores of implicit memory in D2 group were significantly lower than that in D1 group(P < 0.05),In group D3,patient’s implicit memory and explicit memory scores was not statistically and significantly different from zero(P < 0.05),The decline of MAP and HR in D1 group were not obvious after pumping DEX(P > 0.05),MAP and HR in group D2 and D3 have obvious decline after pumping DEX(P < 0.05).The decline in group D3 was more significant than that in group D2(P < 0.05).Conclusion pumping Low dose of DEX could not cause the loss of implicit and explicit memory,while pumping moderate dose of DEX could cause the loss of explicit memory but the patient’s implicit memory still existed,In addition,the inhibition of pumping medium dose of DEX was more obvious than that of pumping low dose of DEX,and in the high dose group,DEX could eliminate the patients’ implicit memory and explicit memory. |
