| Objective: To observe and analyze the ratio of neutrophils to lymphocytes(neutrophil-to-lymphocyte ratio,NLR)in patients with type 2 diabetes mellitus,and to explore the effect of NLR on the pathogenesis of(Diabetic retin-opathy,DR)in diabetic retinopathy.Methods: T total of 254 inpatients with type 2 diabetes mellitus were selected from the second Endocrinology Department of Shengjing Hospital affiliated to China Medical University from January 2016 to December 2019.According to the presence or absence of diabetic retinopathy,the patients were divided into non-diabetic retinopathy group(NDR group)and diabetic retinopathy group(DR group).The DR group was further divided into proliferative diabetic retinopathy group(PDR group)and non-proliferative diabetic retinopathy group(NPDR group).The general data of all patients and indexes such as glycosylated hemoglobin(Glycated hemoglobin,Hb A1c),fasting blood glucose((Fasring plasma glucose,FPG),total cholesterol(Total cholesterol,TC),triglyceride(Triglycerides,TG),low density lipoprotein cholesterol((Low density cholesterol,LDL-C),high density lipoprotein cholesterol((Tigh density cholesterol,HDL-C),urinary microalbumin / creatinine(Urine albumin-to-creatin,UACR)and blood routine were collected.The above results were analyzed by SPSS 23.0 statistical software.Results: 1.Between NDR group and DR group,FPG,Hb A1 c,NLR,TG,incidence of cardio-cerebrovascular disease,UACR,course of disease and systolic blood pressure in DR group were significantly higher than those in NDR group,while vitamin D level was significantly lower than that in NDR group.2.Between NPDR group and PDR group,NLR,hypertension incidence,UACR and course of disease in PDR group were higher than those in NPDR group,while the level of vitamin D was lower than that in NPDR group,and these differences were statistically significant.3.Spearman correlation analysis showed that the level of NLR was positively correlated with the course of disease and TG(r = 0.16,0.13,P < 0.05,0.05),and negatively correlated with HDL(r =0.15,P < 0.05).4.The results of multiple linear regression analysis showed that after excluding the confounding factors such as age,sex and family history,when the course of disease increased by 1 year,NLR increased by 0.2 and HDL decreased by 0.63.5.Binary Logistic regression analysis showed that the course of disease,NLR,vitamin D level,systolic blood pressure and TG were the influencing factors of DR.Conclusion: 1.The results of this study suggest that there is a correlation between NLR and DR.2.High NLR,TG,vitamin D level and systolic blood pressure may be risk factors for DR.3.The increase of disease course and the decrease of HDL can increase NLR,and then affect the occurrence of DR. |
