Objective:To investigate the effects of Hyperuricemia(HUA)on the osteoclast and osteoclast activity of alveolar bone,we compared the levels and ratios of RANKL and OPG in gingival crevicular fluid between Hyperuricemia(HUA)patients and healthy controls.Method:The serum uric acid(SUA)levels and periodontal conditions of newly diagnosed male patients aged 18-35 years in the orthodontic department and gout clinic from 2020 to 2021were screened.Finally,29 untreated patients with HUA were selected as the experimental group,with an average age of 25.4±4.2 years.And 25 volunteers with normal blood uric acid levels(mean age 26.6±3.5 years)as a control group.All subjects were further divided into the following groups:(1)According to the history of gout attack,they were divided into the gout group,the asymptomatic HUA group and the control group;⑵ According to the blood uric acid concentration,the control group was divided into group A(SUA < 360μmol/L)11 people,group B(SUA: 360-420 μmol /L)14 people,group HUA group was divided into group C(SUA: 420-500 μmol/L)9 people,group D(SUA: 420-500 μmol/L)500-580 μmol/L)and E group(SUA > 580 μmol/L).By measuring the epidemiological dental position(11,16,26,31,36,46),the probing depth(PD),the clinical attachment level(AL),the plaque index(PI),and gingival index(GI)to assess periodontal health.Gingival crevicular fluid was collected from patients with relatively healthy periodontal conditions(PD≤3mm,AL≤1mm).The level of uric acid in gingival crevicular fluid was determined by biochemical reagent;Enzyme-linked immunosorbent assay(ELISA)was used to determine the levels of RANKL and OPG in gingival crevicular fluid.SPSS 25.0statistical software was used for analysis.The differences of periodontal clinical indexes,UA,RANKL and OPG in gingival crevicular fluid between HUA group and control group were compared by independent sample t test.The correlation between serum uric acid and periodontal clinical indexes,the levels of UA,RANKL,OPG and RANKL/OPG in gingival crevicular fluid,and the correlation between UA and RANKL,OPG and RANKL/OPG in gingival crevicular fluid were analyzed by Pearson correlation coefficient analysis.The differences of RANKL,OPG and RANKL/OPG among multiple groups were compared by one-way analysis of variance.Result:1.There were no statistical differences in the four periodontal clinical indexes in HUA group(P > 0.05).2.The concentrations of UA and OPG in gingival crevicular fluid in HUA group were significantly higher than those in control group(P<0.05).There were no significant differences in RANKL and RANKL/OPG between the two groups(P>0.05).3.The concentration of SUA in HUA group and all subjects was significantly positively correlated with the concentration of UA and RANKL/OPG in GCF(P<0.05);There was significant positive correlation between SUA concentration and RANKL concentration in all subjects(P<0.05).UA concentration in gingival crevicular fluid was negatively correlated with OPG concentration(P<0.05),and positively correlated with RANKL/OPG(P < 0.05).4.Comparison between the gout group,the asymptomatic HUA group and the control group: the comparison result of SUA was gout group > Asymptomatic HUA group >Control group(P < 0.05);The UA level of gingival crevicular fluid in the gout group was significantly higher than that in the asymptomatic HUA group and the control group(P <0.05),while the uric acid level of gingival crevicular fluid in the asymptomatic HUA group and the control group had no statistical difference(P>0.05);OPG in asymptomatic HUA group was significantly higher than that in control group(P < 0.05).5.Comparison among different concentration groups: OPG increased gradually from group A to group C,and reached the maximum in group C(65.93 ±6.03 ng/m L),which was significantly higher than that in groups A and B(P < 0.05),but decreased in groups D and E.Conclusion :1.The level of uric acid in gingival crevicular fluid may be related to gout symptoms;2.In the male population aged 18-35,the expression of OPG in alveolar bone of asymptomatic HUA patients was higher than that of normal people;3.Serum uric acid at normal or high normal levels may promote the osteogenesis of alveolar bone,but the protective effect will not be further increased when the level of uric acid rises to a higher concentration.
