| Objective To maslow’s hierarchy of needs,welfare pluralism theory,utility theory as the instruction,in 60 years of age or older as investigation object,using the discrete choice experiment,this method analysis the influence factors of the elderly pension preference have combined with institutions,and measure the value of monetary substitution effects between influencing factors in order to examine the influencing factors and differences in preference of older people in institutions combining medical care and care for the elderly among different groups;It provides an important basis and reference for the interpretation,prediction and intervention of the institutionalized care service.Methods First of all,through learning literature and in-depth interviews,in four areas:service form,service type,service mode,etc.,service personnel,service content and service fee of medical and nursing institutions.Secondly,through literature research and expert consultation,the attributes and level of institutional elderly care service are designed.Then,the mixed level orthogonal method was used to determine the combination of the attributes and levels of the institutional care service for the elderly as a questionnaire to design the demand preference of the institutional care service for the elderly and to issue and recycle the questionnaire.Results First,the location of institutions,the form of combination of medical care and nursing care,medical service conditions,the content of elderly care services,whether to be included in the designated institutions of medical insurance and the P value of the charging standard were all <0.01,indicating that these service attributes all affect the preference of elderly care institutions combined with medical care.In addition,the elderly in the choice of medical care institutions,in addition to the economic factor of the fee standard,but also by the medical service conditions,pension service content and other non-economic factors.Second,the impact of these six attributes on the preference of the elderly in institutions combining medical care and old-age care varies in size and effect.The regression coefficient of charging standard,institution site selection and old-age care service content is positive,indicating that these three service attributes have a positive effect on the elderly in choosing institutions combining medical care and old-age care.The regression coefficient of medical insurance designated institutions,type of medical care combination and medical service conditions is negative,indicating that these three service attributes have a negative effect on the selection of medical care combination institutions for the elderly.Third,the elderly groups with different individual characteristics have different pension preferences.Fourth,the monetary value of medical services with good conditions(1,285.35 yuan)is the largest,and the combination of medical care and nursing care is the smallest in the form of cooperation between medical institutions and pension institutions(734.81 yuan).The elderly are willing to give up1,285.32 yuan in order to choose institutions with good medical service conditions rather than institutions with general medical conditions.The elderly are willing to give up part of their economic benefits to obtain the service attributes they like,while the elderly hate the service attributes they hate.If they are allowed to accept these attributes,they must compensate for their economic benefits.Conclusion Discrete selection experiment can be selected as the appropriate method to study the demand preference of the elderly in combination with medical care.The preference of the elderly is influenced by the economic factors such as fee standard,the location of institutions,whether to be included in the designated institutions of medical insurance,medical service conditions,the content of elderly services and the form of combining medical care with nursing care.The effects of different service attributes on the elderly group are also different.Figure 4;Table 24;Reference 50... |
